Members Library Terms
Additional terms for the clinician guides, DSM-5 criteria and sheets, rating scales, patient handouts, and Guide Builder. The information may be incorrect. Check it yourself before you rely on it.
The information in the Members Library may be incorrect, incomplete, out of date, or flatly wrong. It is educational only, it is not medical advice, and nobody has promised you that any of it is accurate. Every dose, interaction, criterion, score, and handout must be checked against official sources before you rely on it. You use the Library at your own risk. Please read these terms carefully.
1. What these terms cover
These Members Library Terms (the “Library Terms”) apply to everything in the Psycho.farm Members Library and to anything you reach from it (together, “Library Content”), including:
- the clinician prescribing guides, including the free preview and the At a Glance panels;
- the DSM-5 criteria search and the one-page DSM-5 sheets;
- the rating-scale and screening calculators and the explanations on those pages;
- the patient handouts, patient resource guides (including the free ones), skill sheets, and book recommendations;
- the Guide Builder and every PDF or other document it produces;
- the Guide Finder app and any other download, tool, or search feature offered with the Library; and
- updates to any of the above.
These Library Terms add to the Terms of Use, the Medical & Educational Disclaimer, and the Privacy Policy. They do not replace them. If something in these Library Terms conflicts with those documents about Library Content, these Library Terms control.
“Psycho.farm Parties” means Prospect Park Psychiatry, PLLC, Psycho.farm, Greg Malzberg, MD, and their owners, members, hosts, co-hosts, authors, editors, instructors, guests, contributors, employees, contractors, licensors, vendors, and agents.
2. How you agree
When you open the Library, a pop-up asks you to read a summary of these terms and to tick a box and click “Agree and continue.” You are asked again each time you start a new visit, and again whenever these terms change in a way that needs fresh agreement. You are not asked again for every page within the same visit.
You agree to these Library Terms when you click, and also by opening or using any Library Content, whether or not the pop-up appeared. Your click is your electronic agreement, and you agree that it has the same effect as a signature to the fullest extent the law allows. If you do not agree, do not use the Library.
You confirm that you are at least 18 years old. If you use the Library for a practice, employer, or other organization, you confirm that you have authority to bind it, and these Library Terms bind both you and that organization.
3. Education only. No advice. No relationship.
Library Content is general educational and informational material. It is not medical, psychiatric, psychological, nursing, pharmacy, or legal advice. It is not diagnosis, treatment, prescribing instruction for any person, supervision, consultation, a second opinion, or a standard-of-care determination. Using the Library creates no clinician-patient, supervisory, consulting, or other professional or fiduciary relationship between you and any Psycho.farm Party.
4. The information may be incorrect
Library Content may contain errors, omissions, typographical mistakes, transcription mistakes, calculation mistakes, outdated statements, and statements that are simply wrong. This can include, without limitation, doses, titration and tapering schedules, cross-tapers, drug interactions, enzyme and transporter effects, contraindications, boxed warnings, monitoring and laboratory recommendations, pregnancy and lactation information, pediatric and geriatric information, renal and hepatic adjustments, diagnostic criteria, specifiers and codes, scoring rules, cut-offs and severity bands, and the instructions in patient handouts.
Psycho.farm Parties make no promise, guarantee, or representation that any Library Content is accurate, complete, current, reliable, internally consistent, or suitable for any purpose or person. Treat all of it as unverified until you have checked it against the official sources in Section 5.
- Summaries are not the source. Where Library Content describes a drug label, a guideline, a diagnostic manual, or a published instrument, it is a shortened, simplified, and sometimes paraphrased description. The official document controls. If Library Content and an official source disagree, follow the official source.
- Things change. Labels, boxed warnings, guidelines, scheduling, shortages, recalls, and diagnostic criteria change after Library Content is written. A date, a “last reviewed” note, or the fact that something is in the Library does not mean it is current.
- Doses and ranges are general. They are not individualized and may be wrong or unsafe for a particular patient.
- No duty to update or correct. Psycho.farm Parties may update, change, remove, or leave Library Content as it is, at any time and without notice, and are not obligated to do any of those things.
If you find a mistake, please email psychofarm333@gmail.com. We will look at it but do not promise a reply or a correction, and reporting a mistake creates no obligation or relationship.
5. You must verify everything
Before you rely on any Library Content for any clinical, prescribing, diagnostic, coding, billing, documentation, legal, teaching, or patient-education purpose, you must check it yourself against current authoritative sources, which may include:
- the current FDA-approved prescribing information and any Medication Guide for each drug;
- the current DSM-5-TR, published by the American Psychiatric Association, and the ICD coding rules that apply to you;
- the official version of each rating scale from its author or publisher, with its official scoring instructions;
- current clinical practice guidelines and primary literature;
- a drug-interaction reference, a pharmacist, or a specialist, where the question calls for one; and
- the law, regulations, and policies that apply to you where you practice.
You agree not to use Library Content as your only source for any decision, and you accept that verifying it is your responsibility, not ours.
6. Your professional judgment and your responsibility
Every clinical, prescribing, diagnostic, documentation, billing, and patient-education decision you make is yours alone. Library Content does not replace your training, licensure, supervision, consultation, institutional policy, or independent clinical judgment, and it does not replace the facts of the patient in front of you.
Library Content is not a clinical practice guideline, protocol, order set, or statement of the standard of care. You will not describe it as one, and you will not cite it as authority in a medical record, to a payer, in a legal proceeding, or in a licensing or credentialing matter. Discussion of off-label use, tapering, switching, augmentation, pregnancy, children, older adults, substance use, and suicide risk is educational only.
You remain responsible for complying with the laws and rules that apply to you, including scope of practice, controlled-substance and prescription-monitoring rules, telehealth rules, informed consent, mandatory reporting, documentation, and privacy. If you are a trainee, you remain responsible to your supervisor and your program, and Library Content is not supervision.
7. Rating scales and calculators
- Software can be wrong. The calculators apply the scoring rules of published instruments in software. A calculator may contain a bug, may implement or describe a rule differently from the instrument’s authors, or may not match the version of the instrument you are licensed to use. Check each score by hand or against the official scoring instructions before you record it, bill for it, act on it, or share it.
- A score is not a diagnosis. A screening score, a severity band, or a change score informs a clinical interview and examination and never replaces them. Cut-offs, sensitivity, and specificity vary by population, setting, language, age, and version, and an instrument may not have been validated for your patient.
- Safety-related tools need extra care. A low or negative score on a suicide-risk screen, a withdrawal scale such as the CIWA-Ar or COWS, or any other safety-related tool does not rule out risk. These tools support, and never replace, clinical assessment and your emergency procedures.
- Not a regulated medical device. The calculators have not been cleared or approved by the FDA or any regulator.
- The instruments belong to others. Each rating scale is the property of its authors, publishers, or copyright holders. Some require permission, a fee, or attribution for certain uses, including use in an electronic record or a commercial practice. Listing a scale in the Library does not give you a license to the instrument. You are responsible for any permission your use requires, and where a tool takes only a score, you must obtain the official form yourself.
- Not a record. The calculators are not an electronic health record and are not designed to receive or store patient information. Do not enter protected health information, and do not rely on the page as the record of a result.
8. DSM-5 criteria and sheets
The DSM-5 criteria search and the one-page sheets are condensed summaries made for quick lookup. They are shortened, may be incomplete or wrong, and may not reflect the latest edition, text revision, coding update, or erratum. They are not the DSM-5 or the DSM-5-TR and do not replace either. Do not diagnose, code, bill, document, write a report, or give an opinion in a legal, disability, or forensic matter from them alone; use the current DSM-5-TR. DSM, DSM-5, and DSM-5-TR are trademarks of the American Psychiatric Association, which has not endorsed or sponsored Psycho.farm. See also Section 14 of the Medical & Educational Disclaimer.
9. Patient handouts and the Guide Builder
- General, not individual. Handouts and patient resource guides are general education. They are not written for any one patient, may not fit your patient, and may contain errors.
- You review before it goes out. You are responsible for reading each handout and each Guide Builder document before it is given to any patient, for deciding whether, when, and how to give it, and for adapting it to the patient’s age, language, reading level, pregnancy status, other conditions, and other medicines. Psycho.farm Parties do not review or approve what you select, generate, print, or hand out.
- It becomes your communication. Once you give a handout or a generated document to a patient, you take responsibility for it as your own patient communication, including any note, name, or instruction you added to it.
- Not a substitute for required steps. A handout does not replace informed consent, counseling, documentation, or the FDA Medication Guide where the law requires one, and it does not tell a patient what to do in an emergency. Make sure your patient also has your contact information and the number to call in a crisis.
- Patient information. Enter as little patient information as possible. Do not enter protected health information into any Library tool beyond what the tool asks for. You are responsible for any patient information you choose to enter and for how you store, print, and share the documents you create.
- Your license. Unless a handout says otherwise, you may give Library handouts and Guide Builder documents to your own patients in the course of your care of them. You may not sell them, post them publicly, or distribute them to others (see Section 13).
10. If you are not a licensed clinician
The Library is made for clinicians and trainees. If you are a patient, family member, caregiver, or other non-clinician, you may read it for general education only. Do not diagnose or treat yourself or anyone else with it, and do not start, stop, increase, decrease, combine, or change any medication or treatment because of anything in it. Talk to your own licensed clinician. We do not know who you are or what your health is, and Library Content is not about you.
11. Emergencies
The Library is not an emergency or crisis service. If you or someone else may be in danger, call 911 or your local emergency number. In the United States, call or text 988 for mental-health crisis support.
12. Assumption of risk, warranty disclaimer, and release
Assumption of risk. You understand that psychiatric medicine involves serious risks, that Library Content may be wrong, and that relying on it could contribute to harm to patients or to you. You choose to use the Library knowing this, and you assume all risk of using it and of any decision you make after using it.
No warranties. To the fullest extent permitted by law, the Library and all Library Content are provided “as is” and “as available,” without warranties of any kind, express or implied. Psycho.farm Parties disclaim every warranty, including accuracy, completeness, timeliness, reliability, clinical usefulness, merchantability, fitness for a particular purpose, non-infringement, and uninterrupted or error-free operation. No advice or information, oral or written, given by a Psycho.farm Party creates a warranty.
Release. To the fullest extent permitted by law, you release and forever discharge the Psycho.farm Parties from every claim, demand, loss, and liability, known or unknown, arising out of or related to your access to or use of, or reliance on, Library Content, including errors or omissions in it, and including claims for personal injury, death, adverse patient outcomes, malpractice or professional-liability claims, licensing or disciplinary matters, and economic loss. This release does not apply to liability that the law does not allow to be released, such as liability for gross negligence or willful misconduct in a state that does not allow it to be waived.
13. License and restrictions
Subject to these terms and to the Terms of Use, we grant you a limited, revocable, non-exclusive, non-transferable license to use Library Content for your own education and for the care of your own patients. You may not share your access; copy, scrape, or bulk download Library Content; sell, resell, publish, post, or redistribute it; use it to build a competing product or database; or use it to train, fine-tune, or prompt any artificial-intelligence system. We own or license Library Content, and all rights not granted are reserved. Library Content may include material owned by third parties, who keep their rights.
14. Limitation of liability and indemnification
The limitation of liability in Section 26 of the Terms of Use and the indemnification in Section 27 apply to Library Content in full, and are repeated here. To the fullest extent permitted by law, no Psycho.farm Party is liable for any indirect, incidental, consequential, special, exemplary, or punitive damages, or for any adverse clinical or patient outcome, patient-care decision, licensing consequence, malpractice claim, disciplinary matter, loss of data, revenue, profit, or goodwill, or other loss arising from the Library or from reliance on Library Content, whatever the legal theory and even if we were told it might happen. To the fullest extent permitted by law, our total liability for any claim relating to the Library will not exceed the greater of (a) the amount you paid for Library access in the 12 months before the claim arose, or (b) $100.
You agree to defend, indemnify, and hold harmless the Psycho.farm Parties from claims, damages, liabilities, losses, costs, and expenses, including reasonable attorneys’ fees, arising from or related to your use of the Library, your clinical or patient-care decisions, any handout or document you give to a patient, your breach of these terms, or your violation of law, professional obligations, or the rights of others.
Some places do not allow certain limits or disclaimers. In those places, our liability is limited to the greatest extent the law allows, and nothing here excludes liability that cannot be excluded by law.
15. Changes
We may change the Library and these Library Terms at any time. The current version, its effective date, and its version number are shown at the top of this page. When a change needs fresh agreement, the pop-up asks you again. Using the Library after a change means you accept the changed terms.
16. Governing law and disputes
Sections 30 (Governing Law), 31 (Informal Resolution), and 32 (Arbitration and Class Waiver) of the Terms of Use apply to these Library Terms and to any dispute about the Library. In short: New York law governs; you agree to try to resolve a dispute informally with us for at least 30 days first; and, subject to applicable law, disputes are resolved by binding individual arbitration in Kings County, New York, and not as a class or representative action.
17. General
If a court finds part of these Library Terms unenforceable, that part is limited as far as needed and the rest stays in effect. Our not enforcing a term is not a waiver of it. These Library Terms, together with the Terms of Use, the Medical & Educational Disclaimer, and the Privacy Policy, are the whole agreement between you and us about the Library. Sections that by their nature should continue after your access ends, including Sections 4, 5, 6, 12, 14, and 16, do.
