Why Clonidine Still Matters
Clonidine is an old antihypertensive that psychiatry adopted, semi-abandoned, and then quietly re-embraced. It is not a first-line ADHD drug and it never should be: stimulants outperform it by a wide margin (effect size roughly 0.5 for clonidine versus 0.8–0.9 for stimulants). But it fills a set of niches that stimulants cannot: the child with ADHD plus tics, the patient with ADHD plus prominent hyperarousal or aggression, the insomnia that stimulants create at bedtime, the opioid or nicotine withdrawal that needs its autonomic storm damped, the PTSD nightmares that nothing else touches cleanly. In those slots clonidine is genuinely useful and often the right call.
It is also a drug that punishes carelessness in two specific ways. First, it drops blood pressure and heart rate, modestly at psychiatric doses, but enough to matter in a frail elder, a child with an unrecognized conduction problem, or anyone stacked on another antihypertensive. Second, and more importantly, it causes rebound hypertension if stopped abruptly, and that rebound can be dangerous. More patients get hurt by clonidine being discontinued wrong than by it being started wrong.
This guide is organized around those realities. The thesis throughout: clonidine is a safe, cheap, versatile adjunct when you start low, use the extended-release formulation for maintenance psychiatric use, check a pulse and blood pressure, and, above all, never let it stop suddenly. Get the taper right and clonidine earns its place in your kit. Get it wrong and you create an emergency out of a benign drug.
A note on nomenclature, because it trips people up: Catapres is the original immediate-release (IR) tablet and the transdermal patch (Catapres-TTS), FDA-approved for hypertension. Kapvay is the extended-release (ER) tablet, FDA-approved in 2010 for ADHD in children and adolescents ages 6–17, as monotherapy or as adjunct to stimulants. Same molecule, different kinetics and different labels. When you write "clonidine" you need to know which one you mean.
Part 1: Indications: Who Is Clonidine For?
FDA-Approved Uses
- Hypertension (Catapres IR tablets and Catapres-TTS patch)
- ADHD in children and adolescents ages 6–17 (Kapvay ER), monotherapy or adjunct to stimulants
Note the boundary that matters clinically: clonidine is not FDA-approved for ADHD in adults. Adult ADHD use is off-label. The pediatric approval also rests on modest data: in the pivotal independent trial (n=122 children), clonidine monotherapy was not superior to placebo on teacher ratings, though it beat placebo on some parent-rated secondary measures, and the stimulant-plus-clonidine combination clearly outperformed placebo.
The Evidence-Based Clinical Uses
ADHD with comorbidity, the real home turf. Clonidine is a third-line ADHD agent on efficacy grounds alone, but it moves up the list when the picture is complicated:
- ADHD + tics / Tourette's. A 2002 study showed clonidine outperforming placebo in ADHD with Tourette's. Alpha-2 agonists don't provoke tics the way stimulants can, and they modestly suppress them. This is one of clonidine's cleanest indications.
- ADHD + aggression, hyperarousal, or explosive behavior. The sedating, arousal-damping profile that is a liability in a purely inattentive child is an asset here.
- ADHD + stimulant-induced insomnia. A small nighttime dose can rescue sleep in a child doing well on a stimulant otherwise: treating two problems with one drug.
- ADHD as a stimulant adjunct. Clonidine (like guanfacine) has evidence for augmenting stimulants: the combination controls symptoms the stimulant alone misses and blunts stimulant side effects. Sedation, which runs high on alpha-2 monotherapy, falls sharply in combination.
Off-label psychiatric uses with reasonable support:
- PTSD-related nightmares and hyperarousal. Clonidine (and its cousin prazosin) target the noradrenergic surge that drives trauma nightmares. Clonidine has more of a psychiatric-comorbidity research base than guanfacine here.
- Opioid, alcohol, and nicotine withdrawal. Clonidine suppresses the autonomic overdrive of withdrawal (the sweating, tachycardia, hypertension, agitation, and piloerection) by damping central sympathetic outflow. This is a classic, well-established use, especially in opioid detox.
- Borderline personality disorder. Small data: PRN clonidine for acute urges (including self-cutting) outperformed placebo.
- Autism-associated irritability, bipolar mania (adjunct), agitation, insomnia, and menopausal hot flashes round out the off-label list.
Think of clonidine not as a weak stimulant substitute but as an autonomic and arousal dial. Whenever the clinical problem is too much noradrenergic tone (tics, hyperarousal, withdrawal storms, trauma nightmares, explosive agitation, stimulant-driven insomnia), clonidine is worth considering. Whenever the problem is pure inattention needing a fast, robust response, reach for a stimulant instead.
Clonidine vs Guanfacine: Choose Deliberately
They are the same class (alpha-2 agonists) and roughly equally effective for ADHD, but they are not interchangeable:
- Guanfacine is more alpha-2A–selective and less sedating. If daytime sedation is the enemy, guanfacine usually wins.
- Clonidine is more sedating, which is exactly why it's the better choice when you want sedation (bedtime dosing for insomnia, agitation, hyperarousal).
- Clonidine has the deeper psychiatric-comorbidity evidence base (PTSD, mania, BPD, autism irritability, withdrawal states).
- Guanfacine has the longer half-life (16–18 h vs clonidine's ~12 h), making true once-daily coverage easier.
A useful heuristic: guanfacine by day, clonidine by night. Reach for guanfacine when you need alpha-2 benefit without sedation across the school or work day; reach for clonidine when the sedation is a feature, not a bug, or when the comorbidity (PTSD, withdrawal, agitation) is one clonidine has been studied in.
Part 2: Before You Start: Workup and Candidacy
Clonidine's workup is light (this is not lithium), but it is not nothing, because the drug acts on the cardiovascular system.
Baseline Assessment

There is no lab requirement to start clonidine. No routine bloodwork, no drug levels.
Who Is a Poor Candidate?
- Significant bradyarrhythmia, sinus node dysfunction, or high-grade AV block
- Symptomatic hypotension, recurrent syncope, or a fall-prone frail elder
- A patient (or family) who cannot be trusted to never stop it abruptly: the rebound-hypertension risk makes reliability a genuine safety prerequisite
- Poorly controlled cardiovascular disease where further BP/HR lowering is hazardous (weigh carefully; coordinate with cardiology)
The most underrated screening question is not medical, it's behavioral: Will this patient or family run out, forget doses, or quit cold turkey? An unreliable patient plus clonidine equals rebound hypertension. If adherence is shaky and no one is supervising, the sedating, forgettable nighttime dose is a setup for exactly the abrupt cessation you fear. Factor this in before you write it.
Part 3: How to Start and Dose
Formulations, and Why the Distinction Is Not Academic

For maintenance psychiatric use, prefer the ER (Kapvay) tablet. It gives smoother levels, fewer peak-related side effects, and is FDA-approved for ADHD. Both ER and IR share roughly a 12-hour half-life, which is why clonidine is typically dosed twice daily even in the ER form, unlike guanfacine ER, which can genuinely go once daily. The weekly transdermal patch is a clever tool for adherence-challenged patients and for minimizing sedation and dry mouth, since it avoids the peaks; it's off-label for psychiatric indications but widely used.
Conversion rule of thumb: 0.3 mg IR ≈ 0.4 mg ER. ER is slightly less bioavailable, so ER doses run a touch higher than the IR equivalent. Do not assume 1:1 when switching formulations.
Starting and Titrating for ADHD (Kapvay ER)
The FDA-labeled ADHD titration is deliberately slow and low:
- Start: 0.1 mg at bedtime (QHS).
- Titrate by 0.1 mg/day each week, splitting toward BID as you go (e.g., week 2: 0.1 mg AM + 0.1 mg QHS).
- Target: 0.2–0.4 mg/day, given in two divided doses. If the split is uneven, give the larger portion at bedtime to keep sedation overnight.
- Maximum: 0.4 mg/day.
- When stopping, taper by 0.1 mg every 3–7 days (see Discontinuation: this is not optional).
Dosing for Other Uses
- Tics / hyperarousal / aggression: similar low ADHD-range dosing, titrated to effect and tolerability.
- Stimulant-induced insomnia: a single small nighttime dose (often 0.05–0.1 mg QHS) is frequently enough.
- Opioid/withdrawal states (inpatient/supervised): higher, more aggressive dosing is used short-term to control autonomic symptoms, with close BP/HR monitoring; this is a different dosing world from chronic ADHD use and should follow a detox protocol.
The Two Governing Dosing Principles
- Dose the sedation where you want it. Clonidine's sedation is its most predictable effect. If daytime somnolence is the problem, shift weight toward the bedtime dose or all of it at night. If you're targeting daytime anxiety or agitation, you'll accept some daytime dosing and manage the drowsiness.
- Effect builds over weeks, not hours. Like all alpha-2 agonists, clonidine's ADHD benefit accrues gradually over 2–5 weeks. Do not judge efficacy, or push the dose impatiently, in the first several days. Set that expectation with families up front, or you'll get premature "it's not working" abandonment.
The commonest rookie error with clonidine ADHD dosing is treating it like a stimulant, expecting a same-day response and escalating too fast when it doesn't come. It's the opposite drug: slow to build, slow to titrate, slow to stop. Patience in all three directions.
Part 4: Monitoring
Monitoring clonidine is refreshingly simple, and it is almost entirely cardiovascular.

That's the core of it. There are no required labs and no drug levels. The whole monitoring burden is a blood-pressure cuff and a pulse.
Additional points:
- Coordinate with the PCP if the patient is on any antihypertensive. Most patients tolerate their existing regimen alongside clonidine, but the combination warrants closer BP checks.
- Watch for orthostasis in the elderly and in anyone reporting dizziness on standing.
- QTc: trials showed slight prolongation with clonidine and guanfacine, enough that 2% of subjects had EKG changes still detectable two weeks post-discontinuation. This is minor for most patients but is a reason to get a baseline ECG in older/frail patients or when co-prescribing other QT-affecting agents.
Part 5: Side Effects and How to Manage Them
Clonidine's side effects are dominated by one theme: it's an alpha-2 agonist, so it sedates and it lowers sympathetic tone. Most effects are dose-related and lessen over the first weeks. Manage them by dose, timing, and formulation.
Sedation / Somnolence / Fatigue: The Signature Effect
This is the effect that makes or breaks tolerability. On alpha-2 monotherapy, sedation is common: guanfacine monotherapy ran 59% in trials, and clonidine is more sedating than guanfacine. The crucial mitigation is combination: paired with a stimulant, sedation drops to around 11%.
Management, in order:
- Shift dosing toward bedtime: put the sedation where it does no harm (and helps sleep).
- Go slower on titration; sedation often attenuates over 1–2 weeks at a stable dose.
- Combine with a stimulant if clonidine monotherapy is too sedating and a stimulant is otherwise appropriate: this is the single most effective anti-sedation move.
- Switch to guanfacine if you need alpha-2 benefit with less sedation.
- Consider the transdermal patch, which flattens peaks and reduces sedation rates.
Cardiovascular (Hypotension, Bradycardia)
Expected pharmacology, not a surprise. Usually modest at psychiatric doses but real, and additive with other antihypertensives.
Management:
- Check HR/BP at every dose change; slow titration if trending low.
- Educate on orthostasis: rise slowly, hydrate.
- Coordinate antihypertensive regimens with the PCP.
- In the independent pediatric trial, clonidine caused more bradycardia and sedation than methylphenidate, so in a child, don't dismiss a low pulse as incidental.
Dry Mouth
Common and bothersome, driven by the reduction in secretions from central sympatholysis.
Management: sugar-free gum/lozenges, hydration, and, most effectively, the transdermal patch, which markedly reduces dry-mouth rates, by avoiding peak concentrations.
Constipation
An alpha-2 class effect.
Management: standard bowel regimen: hydration, fiber, activity, an osmotic laxative if needed.
Rebound / Withdrawal Phenomena on Missed or Stopped Doses
Not a "side effect" of taking it but of not taking it, covered fully in Part 8, but flag it here because a patient who skips doses can experience rebound anxiety, agitation, tachycardia, and hypertension. Irritability and sleep disturbance on missed doses are a signal, not a coincidence.
When a stable clonidine patient suddenly reports feeling wired, anxious, hypertensive, or tachycardic, your first question is not "do we need more" but "did you miss doses?" Rebound sympathetic overshoot masquerades as worsening illness. Chasing it with a dose increase when the real problem is inconsistent adherence sets up a dangerous cycle.
Part 6: Overdose and Toxicity
Clonidine overdose is a real clinical entity, especially relevant because these are often household medications in homes with children, and because pediatric ADHD patients are the population who have it around.
The Clonidine Overdose Picture
Clonidine overdose produces a picture that can superficially mimic opioid toxicity:
- CNS depression / lethargy / coma
- Bradycardia
- Hypotension (though transient early hypertension can occur first from peripheral alpha stimulation)
- Respiratory depression
- Miosis (pinpoint pupils)
Children are exquisitely sensitive: small ingestions can cause significant toxicity, so any pediatric clonidine ingestion is a medical emergency.
Management
Supportive care is the mainstay: airway, IV fluids for hypotension, atropine for symptomatic bradycardia.
- Naloxone is sometimes tried given the opioid-like presentation; response is inconsistent but it's reasonable to attempt in a depressed, apneic patient.
- Most cases resolve with supportive care as the drug clears.
Boxed Warnings
Clonidine carries no FDA boxed warning for its psychiatric or hypertensive use. (This distinguishes it from atomoxetine, the non-stimulant that carries a suicidality boxed warning in youth.) The most serious clonidine-specific safety issues are not boxed but are clinically critical: rebound hypertension on abrupt discontinuation and overdose toxicity, particularly in children.
The two clonidine dangers that actually send people to the hospital are both about the wrong amount at the wrong time: a child getting into the bottle, and any patient stopping suddenly. Counsel on secure storage the same day you counsel on the taper. Both conversations belong at the first visit.
Part 7: Drug Interactions
Clonidine's interactions are pharmacodynamic (additive physiologic effects) far more than pharmacokinetic: it is not a major CYP450 player the way guanfacine (a 3A4 substrate) or atomoxetine (a 2D6 substrate) are.
Additive CNS Depression
- Alcohol, benzodiazepines, opioids, sedating antihistamines, and other sedatives compound clonidine's somnolence and can deepen CNS/respiratory depression. Counsel accordingly and go cautious on doses in patients already on sedatives.
Additive Cardiovascular Effects
- Other antihypertensives (beta-blockers, calcium channel blockers, other central agents): additive hypotension and bradycardia; monitor BP/HR, coordinate with the PCP.
- Drugs that lower heart rate or prolong QTc: additive with clonidine's mild effects, more relevant in the frail or cardiac patient.
If a patient is on both clonidine and a beta-blocker and clonidine is stopped abruptly, the rebound hypertension can be worse than usual, from unopposed alpha stimulation. The classic teaching: stop or taper the beta-blocker first, and never yank clonidine out from under a running beta-blocker.
Interactions That Blunt Clonidine's Effect
- Tricyclic antidepressants (and other agents with strong noradrenergic/alpha activity) can antagonize clonidine's antihypertensive and central effects. Relevant if you're relying on clonidine's BP or arousal-damping action.
Food
- Clonidine absorption is essentially unaffected by food: a practical convenience, and a point of contrast with guanfacine, whose absorption rises (and adverse events may rise) with high-fat meals.
Part 8: Discontinuation: The Part You Cannot Get Wrong
This is the most important section in the guide, because this is where clonidine actually hurts people.
Rebound Hypertension Is the Defining Hazard
Abrupt cessation of clonidine causes a rebound sympathetic surge: the chronic alpha-2 suppression of central noradrenergic outflow is suddenly withdrawn, the system overshoots, and blood pressure and heart rate spike, sometimes to hypertensive-urgency levels, along with anxiety, agitation, sweating, tremor, and insomnia. This is a well-characterized, dangerous, and entirely preventable event. It is more pronounced with clonidine than with guanfacine, since clonidine's shorter half-life and receptor profile make the withdrawal sharper.
How to Stop Clonidine
- Always taper. Never stop abruptly.
- A reasonable pace: decrease by 0.1 mg every 3–7 days. Slower in patients on higher doses, longer duration, or with cardiovascular disease.
- Watch BP and HR during the taper, especially in the first days after each reduction.
- If the patient is on a beta-blocker, remember the interaction above: don't discontinue clonidine while a beta-blocker runs unopposed. Taper clonidine first, or discontinue the beta-blocker first, coordinated with the prescriber.
- Build in refill safety. Because rebound is triggered by running out as easily as by intentional stopping, make sure the patient never lapses. This is where the transdermal patch, pill organizers, and family involvement pay off.
"This medicine has to be stopped slowly and never all at once. If you run out or want to stop, call me first. Stopping suddenly can send your blood pressure dangerously high." Say it when you start the drug, not when you stop it. By the time you're stopping it, the patient who was going to run out already has.
A Note on the ADHD Taper
Even setting cardiovascular rebound aside, the FDA Kapvay labeling itself directs tapering when discontinuing (by 0.1 mg every few days) rather than abrupt cessation. There is no scenario in routine psychiatric practice where stopping clonidine cold is the right move.
Part 9: Special Populations
Pregnancy
Data are limited. Clonidine crosses the placenta and has been used in pregnancy for hypertension. There is no clear major teratogenic signal, but the evidence base is thin and it is not a first-choice psychiatric agent in pregnancy. Weigh the modest, off-label psychiatric benefit against uncertainty; for most psychiatric indications, non-pharmacologic or better-studied options come first. Neonatal effects, including the theoretical risk of neonatal rebound hypertension after in-utero exposure near term, warrant coordination with obstetrics.
Lactation
Clonidine is excreted in breast milk. Given its sedative and cardiovascular effects on an infant, use during breastfeeding is generally approached with caution and infant monitoring; discuss risks and alternatives.
Elderly
The population where clonidine's cardiovascular effects bite hardest. Bradycardia, hypotension/orthostasis, sedation, and QTc effects are all more consequential in older or frail patients, who were underrepresented in the pediatric ADHD trials. Start low, go slow, check orthostatic vitals, consider a baseline ECG, and watch for falls and confusion.
Renal and Hepatic Impairment
- Clonidine is substantially renally eliminated, so dose reduction is prudent in renal impairment: accumulation raises the risk of hypotension and sedation.
- It is not a major CYP substrate, so hepatic impairment is less of a driver than for guanfacine or atomoxetine, but caution and clinical judgment apply.
Pediatrics
This is clonidine's FDA-approved psychiatric home (Kapvay, ages 6–17). Practical points:
- Children have shorter alpha-2 agonist half-lives than adults, reinforcing BID (or all-at-night) dosing.
- Bradycardia and sedation exceeded methylphenidate's in head-to-head pediatric data: monitor pulse, not just behavior.
- Overdose is a pediatric emergency and small ingestions are dangerous: counsel families on secure storage.
- Efficacy is real but modest as monotherapy; the strongest pediatric evidence is for the stimulant-plus-clonidine combination and for the comorbid presentations (tics, aggression, stimulant-induced insomnia).
Part 10: Clonidine vs the Alternatives
vs Stimulants
Stimulants win decisively on efficacy (effect size ~0.8–0.9 vs clonidine's ~0.5) and on speed (hours vs weeks). Clonidine's edge is in what stimulants can't do or shouldn't do: comorbid tics, prominent hyperarousal/aggression, stimulant-induced insomnia, substance-use histories where a non-controlled, non-abusable agent is preferable, and as an add-on to smooth stimulant side effects.
vs Guanfacine
Same class, similar ADHD efficacy. Guanfacine is less sedating, longer-acting (easier once-daily dosing), and a 3A4 substrate (more drug interactions). Clonidine is more sedating (an asset for sleep/agitation/hyperarousal), shorter-acting, and has the richer psychiatric-comorbidity evidence base (PTSD, withdrawal, BPD, autism irritability). Guanfacine by day, clonidine by night.
vs Atomoxetine / Viloxazine
Different mechanism (NE reuptake inhibition). Those agents are non-controlled, once-daily, and without rebound-hypertension risk, but carry a pediatric suicidality boxed warning, take 4–6+ weeks to work, and are more useful for pure inattention. Clonidine wins when you specifically need arousal-damping or sedation, or the comorbidities it's studied in.
vs Prazosin (for PTSD Nightmares)
Prazosin (an alpha-1 antagonist) is the more established, better-studied choice specifically for trauma nightmares; clonidine is a reasonable alternative, particularly when its sedation and broader autonomic effects are also wanted.
A cheap, generic, non-controlled, non-abusable alpha-2 agonist whose sedation and sympatholysis are its selling points, best deployed as an adjunct or in comorbid presentations, not as a stand-alone answer to uncomplicated ADHD.
Mechanism: The Short Version
Clonidine is a central alpha-2 adrenergic receptor agonist. By stimulating presynaptic alpha-2 autoreceptors (concentrated in the brainstem and locus coeruleus), it reduces central sympathetic (noradrenergic) outflow. That single action explains nearly everything about the drug:
- Lowered sympathetic tone → reduced blood pressure and heart rate (its original antihypertensive use), and suppression of the autonomic overdrive of opioid/alcohol/nicotine withdrawal.
- Sedation and anxiolysis → from dampened locus coeruleus firing and reduced central arousal.
- ADHD benefit → the more nuanced part. In the prefrontal cortex, alpha-2A stimulation is thought to strengthen the noradrenergic signaling that supports attention and impulse control, the same postsynaptic mechanism guanfacine exploits more selectively. So clonidine both damps global sympathetic tone and tunes prefrontal signaling, which is why it helps attention and behavior despite being "a sedative."
Half-life is roughly 12 hours for both IR and ER, driving twice-daily dosing. It is largely renally eliminated and is not a significant CYP450 substrate. Its structural analog is xylazine, worth knowing in the current era of xylazine-adulterated street opioids, since it shares clonidine's alpha-2 sedative/bradycardic profile.

Know your formulation
- Kapvay (ER) = ADHD, ages 6–17, BID.
- Catapres (IR/patch) = hypertension.
- 0.3 mg IR ≈ 0.4 mg ER.

The rules you cannot break
- NEVER stop abruptly → rebound hypertension. Taper 0.1 mg every 3–7 days.
- Never yank clonidine out from under a beta-blocker (worse rebound).
- Secure storage: pediatric overdose is an emergency (lethargy, bradycardia, miosis, respiratory depression; supportive care ± naloxone).
- "Feeling wired/anxious/hypertensive"? Ask about missed doses before raising the dose.
Positioning
- Third-line for pure ADHD; first-tier consideration for ADHD + tics, hyperarousal, aggression, stimulant insomnia, or as a stimulant adjunct.
- Off-label workhorse for PTSD nightmares, withdrawal states, agitation.
- Guanfacine by day, clonidine by night.
Clonidine is a modest drug asked to do modest things, and within that scope it is quietly excellent: cheap, generic, non-abusable, and uniquely suited to the patient whose problem is too much arousal rather than too little focus. It will never out-treat a stimulant for uncomplicated ADHD, and it doesn't need to. Its value lives in the niches: the tic-burdened child, the trauma-driven nightmares, the withdrawal storm, the stimulant that works except at bedtime. Respect the two things that actually cause harm (abrupt discontinuation and pediatric ingestion), and clonidine is one of the safest, most useful adjuncts in psychiatry. Forget the taper, and a benign old antihypertensive becomes an avoidable emergency. Everything important about prescribing it follows from that single asymmetry.
