Why Desipramine Still Matters

Desipramine is the tricyclic you reach for when the newer drugs have quietly failed and you need real noradrenergic firepower with the fewest strings attached. It is the most selective norepinephrine reuptake inhibitor of the classic tricyclics, the least anticholinergic of the group, and one of the two secondary amines (with nortriptyline) that were bred specifically to keep the efficacy of the old tertiary-amine tricyclics while shedding much of their dry-mouth-constipation-confusion tax. Structurally, it is the active metabolite of imipramine (imipramine minus a methyl group), which is why it behaves like imipramine's cleaner, more norepinephrine-focused cousin.

Here is the case for keeping it in your armamentarium. When you lump all depressions together, TCAs are as effective as SSRIs, full stop. But depression is not one disease, and in the subtypes where they pull ahead, melancholic depression, depression with chronic pain, treatment-resistant depression, the tricyclics can be genuinely superior to SSRIs. Desipramine is a legitimate Stage 3 option in treatment-resistant depression, the point at which a patient has failed a single-mechanism and then a dual-mechanism antidepressant and you are looking for something that works differently. And because desipramine has a validated serum level, one of only three TCAs that do, you can dose it by measurement rather than by guesswork, which is a luxury you do not have with an SSRI.

Yet it is prescribed less and less, and the reasons have almost nothing to do with efficacy. TCAs carry a "nuisance factor": they want a baseline ECG, they reward serum-level monitoring, and, the fact that dominates every conversation about them, they can kill in overdose. A tricyclic overdose is a cardiology emergency, and in a field that treats a lot of suicidal people, a drug that is lethal in overdose earns a reputation that outlives its usefulness.

This guide is about using desipramine so that its strengths are available to you and its one genuinely dangerous property is systematically contained.

The thesis of this guide

Desipramine is a well-tolerated, level-guided, noradrenergically potent antidepressant whose principal risk, cardiotoxic overdose, is entirely a dispensing-and-selection problem, not a reason to avoid the drug in the right patient. Pick the right patient, get an ECG, dose by level, and don't hand a lethal quantity to someone who might use it that way. Do those four things and you have access to an antidepressant that reaches patients other drugs miss.


How to Use Desipramine With Confidence

Let's be honest about why clinicians skip past the tricyclics. It is not that they've weighed the evidence and found them wanting. It's that a TCA feels like more than it's worth: an overdose that can be fatal, an ECG to order, a blood level to interpret, and a patient population that already includes a lot of suicidal people. If that hesitation is what keeps desipramine off your prescription pad, this section is for you.

The reassuring truth: every one of those concerns is manageable with a short, fixed routine, and none of them requires anything exotic. Desipramine is arguably the most forgiving tricyclic to run: low anticholinergic burden, a clean serum level to steer by, and a dose-response relationship that actually rewards you for titrating.

The Three Real Hesitations, Answered

"It's lethal in overdose, and my patients are depressed."

This is the legitimate one, and it deserves a real answer rather than reassurance. Tricyclics are among the most dangerous antidepressants in overdose because of their type 1A antiarrhythmic (sodium-channel-blocking) effect on the heart: a large ingestion widens the QRS, drops the blood pressure, and can cause fatal arrhythmias and seizures. You do not solve this by avoiding the drug; you solve it by controlling the quantity in the patient's hands. For anyone with meaningful overdose risk, dispense small amounts, a week or two at a time, and involve family in holding the supply. A month's supply of desipramine is a lethal quantity; two weeks generally is not the same order of risk, and a patient who has to come back for refills is a patient you are seeing more often. The counterpoint: desipramine's therapeutic target sits well below toxic concentrations, so ordinary treatment is safe. It is the deliberate large ingestion you are engineering against, and quantity limits plus a partnered family are how you engineer against it.

"I have to order an ECG and I don't know what I'm looking for."

Get a baseline ECG before starting, especially in anyone over 40 or with any cardiac history. You are looking for a few specific things: a QTc that is already prolonged, a QRS that is already widened (a bundle branch block or other conduction delay), and evidence of significant conduction disease. TCAs have type 1A antiarrhythmic properties, they slow cardiac conduction, so a heart that already conducts slowly is the one to worry about. A normal baseline ECG in a patient without cardiac history is your green light. If the tracing is abnormal or you're unsure, that's a one-line curbside to cardiology, not a reason to abandon the plan. Repeat the ECG if you push to high doses or the patient develops cardiac symptoms.

"The serum level thing feels like a hassle."

It is the opposite of a hassle: it's the feature that makes desipramine easier to dose than most antidepressants. Metabolism varies enormously between people: a well-documented 5- to 10-fold difference in blood level at the same dose. That is why a patient can be supratherapeutic on 75 mg and another subtherapeutic on 200 mg. Instead of titrating blind, you titrate to a number. Start at 50 mg, go to 100 mg after a week or two, then draw a level (8 to 12 hours after the dose, at least 5 to 6 days after reaching that dose) and let the number tell you whether to go up. A level above roughly 125 ng/mL is associated with better outcomes. The level also confirms adherence, which is quietly one of the most useful pieces of information you can have in treatment-resistant depression.

The System

Select well. Desipramine is for unipolar depression, ideally melancholic, treatment-resistant, or comorbid with chronic pain. It is not for bipolar depression (mania/mixed-state switch risk) or borderline personality disorder (disinhibition plus overdose lethality). Screen those out and most of the risk disappears with them.

Baseline ECG. Before the first dose in anyone over 40 or with cardiac history; reasonable in most adults given the drug's conduction effects.

Start low, go slow, dose by level. 50 mg to 100 mg after 1 to 2 weeks, then check a trough level, then titrate to the level and the response.

Partner with the patient. Explain the level (it steers dosing and proves the medicine is being taken), the overdose danger (so they respect the pill count), and the early side effects (dry mouth, lightheadedness on standing) that fade as the body adjusts.

Pearl

The blood level does double duty. It tells you where to aim the dose in a patient whose metabolism you can't predict, and it tells you, quietly, non-accusatorially, whether the patient is actually taking the drug. In a treatment-resistant patient who "isn't responding," a subtherapeutic level on a supposedly adequate dose reframes the whole problem: you may be looking at non-adherence or rapid metabolism, not true resistance. Check the level before you call desipramine a failure.

The mindset shift

Desipramine isn't dangerous because it's a tricyclic. It's dangerous in one specific, avoidable scenario, a large deliberate overdose, and completely manageable in every other. Ordinary daily treatment sits far below the toxic range. Order the ECG, dose to the level, limit the quantity, and you have a potent noradrenergic antidepressant that reaches the melancholic, pain-burdened, and treatment-resistant patients whom the SSRIs so often leave behind.

Part 1: Indications, Who Is Desipramine For?

FDA-Approved Use

  • Depression.

That is the label. Everything else is evidence-based off-label use, which, for the tricyclics, is where much of their value lives.

The Evidence-Based Clinical Uses

Melancholic depression, where TCAs earn their keep

This is the classic severe, "endogenous" depression: pervasive loss of pleasure, ruminating guilt, appetite and weight loss, early-morning awakening, and prominent psychomotor change. In melancholic depression, TCAs respond better than SSRIs, one of the few places in the antidepressant literature where the older drugs clearly pull ahead. If a patient's depression has this endogenous, vegetative, psychomotor character and they have not done well on serotonergic agents, desipramine belongs in the conversation.

Treatment-resistant depression (TRD)

Desipramine is a legitimate Stage 3 option, the point at which a patient has failed a single antidepressant and then a second of a different class. Its noradrenergic mechanism is genuinely different from the serotonergic drugs that most patients have already cycled through, which is the whole rationale for switching mechanism in TRD. Tricyclics (and MAOIs) are underprescribed in treatment-resistant depression; desipramine, with its favorable tolerability and steerable serum level, is one of the more usable of them.

Pearl

Before you label a patient "resistant," ask whether they ever had an adequate tricyclic trial: adequate dose, adequate duration, and a documented therapeutic serum level. A great deal of apparent tricyclic failure is really under-dosing in a fast metabolizer, or non-adherence, both of which the serum level will expose. "Failed desipramine" at a subtherapeutic level is not a failed trial.

Depression with chronic pain

TCAs relieve neuropathic pain through a mechanism partly independent of their antidepressant effect, acting on descending serotonergic and noradrenergic pain pathways, with a number-needed-to-treat around 3 for neuropathic pain, superior to SSRIs and comparable to duloxetine. When a depressed patient also carries neuropathic pain, fibromyalgia, or chronic headache, a tricyclic can treat both problems with one drug. Note that for pain specifically, amitriptyline and nortriptyline have the deeper track record; desipramine is a reasonable, better-tolerated choice when a secondary amine is preferred and anticholinergic burden matters.

Augmentation of an SSRI

Adding a noradrenergic tricyclic to a serotonergic SSRI is a strategy that dates to the 1980s: the SSRI covers serotonin, the TCA adds norepinephrine. Desipramine suits this because its flexible (non-window) dose-response gives you room to titrate, but see the interactions section, because SSRIs can push tricyclic levels up dangerously, and this combination demands level monitoring.

The Dose-Response Point That Separates TCAs From SSRIs

Here is a fact worth internalizing: unlike SSRIs, where pushing the dose mostly buys side effects rather than response, for tricyclics higher doses genuinely correlate with greater efficacy (a property they share with venlafaxine and the MAOIs). This is why the serum level matters, and why titrating a tricyclic to an adequate level, rather than settling at a low "starter" dose, is often the difference between response and pseudo-resistance.

Who It Is NOT For

  • Bipolar disorder / bipolar depression. TCAs rank near the top of all antidepressants for inducing mania and mixed states. Do not use desipramine in bipolar depression without a mood stabilizer on board, and prefer other options even then.
  • Borderline personality disorder. Two strikes: risk of triggering aggression and disinhibition, and lethality in overdose in a population with elevated impulsive self-harm.
  • The acutely suicidal patient with access to a lethal supply. Not an absolute contraindication, but if you prescribe it, you control the quantity (see below).

Part 2: Before You Start, Workup and Candidacy

A baseline serum level is not needed (you have nothing to measure yet); the level becomes useful once you reach 100 mg and steady state.

Who Is a Poor Candidate?

  • Recent myocardial infarction or acute cardiac instability
  • Significant cardiac conduction disease / baseline QRS widening or QT prolongation
  • Untreated narrow-angle glaucoma (anticholinergic effect can precipitate an attack)
  • Uncontrolled seizure disorder (TCAs lower the seizure threshold)
  • Bipolar depression without a mood stabilizer
  • Borderline personality disorder with impulsive self-harm
  • High acute overdose risk without a way to control the quantity dispensed

None of these is subtle, and screening for them at the outset is most of what makes desipramine safe.


Part 3: How to Start and Dose

Formulations

Desipramine is available as oral tablets, in the standard tricyclic strengths (10, 25, 50, 75, 100, and 150 mg). This range makes the fine titration below easy to execute. There is no meaningful advantage to a particular brand; generic desipramine is inexpensive.

Starting Dose and Titration

  • Start: 50 mg/day. Give it in the evening, the modest sedation and anticholinergic effects are better slept through, and once-daily dosing is easy on adherence. (Desipramine is among the less sedating tricyclics, so daytime dosing is tolerable for some, but evening is the default.)
  • Increase to 100 mg/day after 1–2 weeks, as tolerated.
  • Then check a serum level, and titrate further based on the level and the clinical response.
  • Titrate slowly. Steps every 5–7 days improve tolerability. Fast escalation buys you orthostasis, dry mouth, and a patient who quits.
  • Typical effective range: most patients land somewhere in the 100–200 mg/day territory, but the right dose is the one that puts the serum level in range for that patient's metabolism, which may be well outside the average.
  • Maximum: the PDR lists 300 mg/day; experienced clinicians have gone to ~500 mg/day in genuinely treatment-refractory cases under close monitoring and level guidance. Do not freelance to those doses without a serum level and an ECG.
Pearl (the O'Reardon approach)

"I like desipramine because it's well tolerated and you have more flexibility in ultimate dosing than a window-type agent like nortriptyline. I start at 50 mg/day, increase to 100 after a week or two, then I check a blood level, and decide on further increases based on that. I get blood levels on all my patients on TCAs because it confirms compliance, and also because there's such a wide range of metabolizers. I've seen patients on 75 mg of desipramine who were supratherapeutic and others on 200 mg who were subtherapeutic." This is the whole philosophy in one paragraph: start low, get to a working dose, then let the number drive.

The Serum Level: Desipramine's Superpower

Desipramine is one of only three tricyclics with a clinically validated serum level (the others are nortriptyline and imipramine). Use it.

Unlike nortriptyline, which has a U-shaped "therapeutic window" where too-high levels lose efficacy, desipramine behaves more as a straightforward threshold: get above ~125 ng/mL and titrate to response and tolerability, watching that you don't run into side effects or toxicity at the top end.
  • When to draw: 8–12 hours after the last dose, and at least 5 half-lives (5–6 days) after reaching the current dose so the level reflects steady state.
  • Why it matters, restated: 5- to 10-fold interindividual variability in metabolism means the milligram number on the bottle tells you very little about what's actually in the blood. The level tells you (a) whether the patient is under-dosed, (b) whether they're taking it at all, and (c) whether they're heading toward a toxic concentration.
Pearl

The therapeutic-level data come mostly from studies in melancholic depression, the population in which most tricyclic serum-level research was done. Treat >125 ng/mL as a well-supported guide rather than a hard universal law, but do draw the level, because the alternative (dosing blind across a 10-fold metabolic range) is worse.


Part 4: Monitoring

Desipramine's monitoring is lighter than lithium's, no renal or thyroid surveillance, but the cardiac and serum-level pieces are non-negotiable.

Act on these

  • Subtherapeutic level with poor response → increase the dose (or address adherence) before concluding the drug failed.
  • New palpitations, syncope, or presyncope → ECG; look for QRS widening or QT prolongation.
  • New elevated mood, decreased sleep need, activation → consider an emerging manic switch; reassess the diagnosis.
  • Confusion (especially elderly) → consider anticholinergic effect; reduce the dose.

Part 5: Side Effects and How to Manage Them

The governing principle mirrors the rest of the class: most desipramine side effects are dose-related, many ease over 2–4 weeks as the patient acclimates, and slow titration is your single best preventive. Desipramine's advantage is that, as the least anticholinergic tricyclic and a secondary amine, it starts from a gentler baseline than amitriptyline or imipramine.

Anticholinergic (dry mouth, constipation, urinary retention, blurred vision, confusion)

Lowest of the tricyclics, but not zero.

Management

  • Dry mouth: sugar-free gum or lozenges, frequent sips of water. Mind the teeth: chronic dry mouth accelerates dental decay.
  • Constipation: increase fluids and fiber, add a stool softener; escalate before it becomes an obstruction, which is a real risk in the elderly.
  • Urinary retention: watch in men with BPH; can precipitate retention and secondary UTI.
  • Blurred vision: usually tolerated and self-limited; caution with narrow-angle glaucoma.
  • Confusion / cognitive fog: more of an issue in the elderly; reduce the dose. Remember the class-wide signal that anticholinergic burden is associated with increased dementia risk, a reason to favor the least anticholinergic agent, which is exactly where desipramine sits.

Cardiovascular

  • Orthostatic hypotension and falls (via α1-adrenergic blockade): the most clinically important cardiovascular effect day-to-day, and more pronounced in the elderly. Start low, titrate slowly, check orthostatic BP, and coach the patient to change positions slowly and stay hydrated. Secondary amines like desipramine cause less orthostasis than the tertiary amines, but it still happens.
  • Cardiac conduction: the type 1A antiarrhythmic effect can slow conduction, widen the QRS, and prolong the QT at higher levels. This is generally benign at therapeutic doses in a structurally normal heart and dangerous in overdose or in a diseased conduction system, hence the baseline ECG and caution in cardiac patients.

Sedation and Weight Gain

Both are less than with the tertiary amines. Desipramine is one of the more activating / less sedating tricyclics; if sedation does occur, dose in the evening. Weight gain is generally milder than amitriptyline or imipramine but can occur.

Sexual Dysfunction

A secondary-amine quirk: desipramine is less likely to cause anorgasmia than the tertiary amines but more likely to cause erectile dysfunction. Ask directly: patients rarely volunteer it, and it's a common silent reason for stopping. Manage with dose reduction, timing, or targeted augmentation if the drug is otherwise working.

Neurologic

  • Seizure threshold: TCAs lower it. Caution in anyone with a seizure history or a traumatic brain injury.

Hepatic

  • Hepatotoxicity is extremely rare (~4 per 100,000 patient-years across the class); routine LFT monitoring is not required.

Part 6: Overdose and Toxicity, Respect This

This is the section that governs how you prescribe desipramine, so know it cold.

The Danger

Tricyclics are among the most lethal antidepressants in overdose. The mechanism is the type 1A antiarrhythmic (sodium-channel-blocking) effect: in a large ingestion, cardiac conduction slows, the QRS widens, myocardial contractility drops, and the patient is at risk of malignant arrhythmias, refractory hypotension, and cardiac arrest. The classic overdose triad is the "three C's": cardiotoxicity, convulsions (seizures), and coma, often with anticholinergic delirium layered on top.

What This Means Clinically

Overdose = emergency department and critical care, full stop

A tricyclic overdose is an emergency-department and critical-care problem. It is not something you manage in the office.

  • The key toxicity marker is QRS widening on ECG: the wider the QRS, the higher the risk of seizures and ventricular arrhythmias. The mainstay of hospital treatment is IV sodium bicarbonate, which narrows the QRS and stabilizes the myocardium, alongside supportive and critical care.
  • The therapeutic-to-toxic margin is narrower than the SSRIs': desipramine has a narrow therapeutic index, in the same conceptual family as lithium and carbamazepine. Therapeutic dosing is safe; a hoarded month's supply taken at once is potentially fatal.

The Boxed Warning

FDA boxed warning: suicidality

Like all antidepressants, desipramine carries the class boxed warning for increased suicidal ideation and behavior in children, adolescents, and young adults (up to age 24). Monitor especially closely early in treatment and after dose changes. This is a distinct issue from overdose lethality but points in the same practical direction: attention to suicidality, and control of the supply.

The Prescribing Rule That Falls Out of All This

Dispense only small quantities to any patient at meaningful overdose risk, a week or two at a time, and involve family in safeguarding the medication. This single practice converts desipramine's most dangerous property into a controlled one. It is the tricyclic-prescribing equivalent of respecting lithium's window: the danger is real, and the containment is simple.

Pearl

When you weigh "toxic in overdose" against reaching for desipramine, separate the two questions it collapses. Question one: is this patient likely to make a serious attempt? If yes, the answer may be a safer drug, or desipramine with a tightly controlled supply and family involvement. Question two: is desipramine dangerous as prescribed and taken correctly? No, therapeutic dosing sits well below the toxic threshold. Don't let the overdose profile scare you out of the drug for a patient who isn't at high overdose risk in the first place.


Part 7: Drug Interactions

Because desipramine has a narrow therapeutic index and a validated level, interactions that move that level matter more than they would for an SSRI.

Drugs That RAISE Desipramine Levels (↑ toxicity risk)

Desipramine is metabolized largely by CYP2D6 (with contributions from other hepatic enzymes). Potent CYP2D6 inhibitors can push the level up substantially.

  • Fluoxetine and paroxetine: potent CYP2D6 inhibitors; can markedly raise desipramine levels. Fluoxetine's effect is prolonged by its long half-life.
  • Fluvoxamine, bupropion, quinidine, cimetidine: other notable inhibitors that raise TCA levels.
  • Practical rule: when adding one of these to desipramine (or vice versa), start low, go slow, and recheck the serum level ~2 weeks later. When augmenting an SSRI with desipramine, remember the SSRI may itself be inflating the tricyclic level, check it.
The SSRI-augmentation exceptions worth memorizing

The following do not meaningfully raise tricyclic levels, which makes them the safer partners when you're combining with desipramine: citalopram, escitalopram, desvenlafaxine, mirtazapine, trazodone, vilazodone, and vortioxetine. If you need a serotonergic partner for a noradrenergic tricyclic, reach for one of these before fluoxetine or paroxetine.

Pharmacodynamic Interactions

  • Other serotonergic drugs → serotonin syndrome risk (lower with desipramine than with the strongly serotonergic tricyclics like clomipramine, but not zero when combined). Watch for agitation, tremor, hyperthermia, rigidity.
  • MAOIs → the classic dangerous combination; do not co-prescribe, and observe an adequate washout when switching.
  • Other anticholinergics / antihistamines → additive anticholinergic burden.
  • Sympathomimetics (pseudoephedrine and the like) → risk of a hypertensive response.
  • Alcohol and other CNS depressants → additive sedation.
  • QT-prolonging drugs → additive conduction effects; be cautious stacking desipramine with other QT-prolongers.

Part 8: Special Populations

Pregnancy

The distilled notes for desipramine don't detail pregnancy, so this leans on established TCA pharmacology: the tricyclics are among the better-characterized antidepressants in pregnancy by virtue of decades of use, without a well-established major-malformation signal at therapeutic doses. As with all psychotropic decisions in pregnancy, the governing consideration is the risk of untreated depression, which carries real harm for both mother and fetus, weighed against drug exposure. A few practicalities:

  • If desipramine is keeping a patient with severe, recurrent depression well, continuing it is often the right call; don't reflexively stop an effective drug in a high-relapse-risk patient.
  • Physiologic changes of later pregnancy can lower drug levels; the serum level is genuinely useful here to keep the patient in therapeutic range, check it if the clinical picture shifts.
  • Near delivery, watch the neonate for jitteriness and anticholinergic or withdrawal-type symptoms; coordinate with obstetrics and pediatrics.
  • Decisions should be individualized and, where possible, made in shared decision-making with the patient and her obstetric team.

Lactation

Not detailed in the desipramine notes. General TCA practice: tricyclics are excreted into breast milk in low amounts and are generally considered relatively compatible with breastfeeding under monitoring, but the decision should be individualized with pediatric input. Don't extrapolate beyond that from these notes.

Elderly

The population where desipramine's low-anticholinergic profile is most valuable, and where its cardiovascular effects most demand respect.

  • Falls and orthostatic hypotension are the dominant concern; start lower, titrate more slowly, check orthostatic BP.
  • Anticholinergic effects (confusion, constipation, urinary retention) are better tolerated with desipramine than with amitriptyline, but the elderly are more vulnerable to all of them, and the anticholinergic-dementia association argues for the least anticholinergic agent, i.e., desipramine or nortriptyline over the tertiary amines.
  • Nortriptyline is the classic first choice in fall-risk elderly (least orthostatic hypotension of the tricyclics); desipramine is a reasonable secondary choice, with the added benefit of dosing flexibility from its non-window serum level.
  • Get the baseline ECG (the age >40 threshold obviously applies) and use serum levels to avoid over-shooting in a population with slower metabolism.

Renal / Hepatic Impairment

Not detailed in the desipramine notes. From general principles: desipramine is hepatically metabolized, so significant liver disease impairs clearance and argues for lower doses and level monitoring; severe liver disease is a caution. Renal considerations are less prominent than for a renally cleared drug like lithium, but frailty and comorbidity in impaired patients warrant the usual start-low, monitor-the-level approach. Don't fabricate specific dose adjustments the source doesn't support, dose to the serum level.

Pediatric

  • The boxed warning for increased suicidality in children, adolescents, and young adults applies.
  • TCAs in youth carry the additional historical concern of cardiac conduction effects; use is uncommon and demands careful monitoring, an ECG, and specialist involvement. (Notably, controlled trials never established robust efficacy for TCAs in pediatric depression.)
  • This is not a first-line pediatric choice; if used at all, it is a considered decision with close surveillance.

Part 9: Discontinuation

Desipramine is not among the worst offenders for discontinuation syndrome. TCAs generally cause less withdrawal than the short-half-life SNRIs (venlafaxine, duloxetine, desvenlafaxine) and the stickiest SSRIs (paroxetine, fluvoxamine). But don't stop it abruptly.

  • Taper gradually rather than stopping cold. Abrupt cessation of a tricyclic can produce a cholinergic-rebound withdrawal (nausea, malaise, sweating, insomnia, agitation) and, less commonly, a withdrawal-emergent dyskinesia within roughly 4–8 weeks of stopping.
  • The other reason to taper is the same as for any antidepressant: to reduce the risk of depressive relapse. It is easier to prevent a relapse than to re-treat a full episode.
  • Pace the taper to the patient: a stepwise reduction over weeks, slowing further if withdrawal symptoms or early relapse signs emerge, and with a plan to resume promptly if the depression returns.

Part 10: Desipramine vs the Alternatives

vs. SSRIs. Equivalent for depression-in-general, but desipramine (and the TCAs) pull ahead for melancholic depression and depression with chronic pain, and offer a genuinely different mechanism for treatment resistance. The trade-offs: SSRIs are far safer in overdose, carry minimal anticholinergic burden, and need no ECG or level. Reserve desipramine for the situations where its edge is real.

vs. Amitriptyline / imipramine (tertiary amines). Desipramine is the least anticholinergic tricyclic, with less sedation, less weight gain, and less orthostasis than the tertiary amines, better tolerated, at the possible cost of the extra serotonergic action those drugs carry. For melancholic depression and pain, amitriptyline has the longer track record; for tolerability, desipramine wins.

vs. Nortriptyline (the other secondary amine). The closest comparison, and largely a choice between two good options. Nortriptyline is the go-to in fall-risk elderly (least orthostatic hypotension) and has a tighter, better-validated U-shaped therapeutic window (50–150 ng/mL). Desipramine's advantage is dosing flexibility: its threshold-type level (>125 ng/mL) rather than a narrow window means you have more room to push the dose in a refractory patient without falling off an efficacy cliff. That flexibility is exactly why some TRD clinicians prefer it.

vs. SNRIs (venlafaxine, duloxetine). SNRIs deliver dual reuptake inhibition with far more overdose safety and no ECG requirement, which is why they've largely displaced TCAs. Where desipramine can still win: melancholic and treatment-resistant depression, and cost. For neuropathic pain, TCAs (NNT ~3) actually edge out duloxetine (NNT ~5).

vs. MAOIs. MAOIs sit further down the treatment-resistant algorithm and outperform TCAs in atypical depression; a meaningful fraction of patients who fail tricyclics respond to an MAOI. Desipramine is generally the earlier move (Stage 3) before the dietary and interaction complexity of an MAOI.

So why is desipramine underused? The same forces that sideline every tricyclic: the overdose lethality, the "nuisance factor" of ECGs and levels, aggressive marketing of newer branded agents, and the lazy assumption that newer is better. None of these is a reason to withhold it from the melancholic, pain-burdened, or treatment-resistant patient who needs what it does.

The Carlat verdict on the class, which fits desipramine exactly

"Tricyclics are rarely first line, but they are a good option in select situations, such as melancholic and treatment-resistant depression, depression with chronic pain, and OCD. With careful management and judicious selection, they are often better tolerated than their reputation suggests."


Mechanism: The Short Version

Desipramine is a secondary-amine tricyclic and a relatively selective norepinephrine reuptake inhibitor, the most NE-selective of the classic tricyclics. It has only weak serotonin reuptake inhibition, which is precisely what distinguishes it from its parent compound imipramine (a balanced serotonin-and-norepinephrine reuptake inhibitor); desipramine is the demethylated, norepinephrine-focused metabolite of imipramine. Layered on top of reuptake inhibition are the receptor effects that generate its side-effect profile: α1-adrenergic blockade (orthostatic hypotension), H1-antihistamine activity (some sedation), and muscarinic anticholinergic activity (dry mouth, constipation, the lowest of the tricyclics). Its type 1A antiarrhythmic action on cardiac sodium channels is the property that makes it dangerous in overdose. Metabolism is hepatic, principally via CYP2D6, with a half-life on the order of ~15–24 hours that supports once-daily dosing and reaches steady state in about 5–6 days, the interval that governs when you draw the serum level.

Quick Reference

Starting

  • Generic desipramine tablets. Start 50 mg (evening), 100 mg after 1–2 weeks, check a serum level, titrate to level + response.
  • Titrate in steps every 5–7 days. Typical effective range ~100–200 mg/day; PDR max 300 mg (higher only with level + ECG in refractory cases).
  • Baseline ECG (esp. age >40 or cardiac history); baseline orthostatic vitals.

Serum level, use it

  • Target >125 ng/mL (threshold, not a narrow window, more dosing flexibility than nortriptyline).
  • Draw 8–12 h after dose, ≥5–6 days after reaching the current dose.
  • The level guides the dose in an unpredictable metabolizer and confirms adherence.

Monitoring

  • ECG at baseline and if pushing high doses or new cardiac symptoms.
  • Serum level after reaching 100 mg, after each dose increase, and ~2 weeks after adding a CYP2D6 inhibitor.
  • Orthostatic BP, anticholinergic symptoms, weight, and watch for a manic switch.

Side effects

  • Least anticholinergic TCA, but still: dry mouth, constipation, urinary retention → fluids, fiber, stool softener.
  • Orthostatic hypotension → start low, titrate slow, slow position changes.
  • Sexual: less anorgasmia, more erectile dysfunction, ask directly.

Overdose, the master caution

  • Fatal in overdose: type 1A cardiotoxicity → QRS widening, arrhythmia, seizures, coma. Hospital treatment is IV sodium bicarbonate + supportive care.
  • Dispense small quantities (1–2 weeks) to any at-risk patient; involve family.
  • Class boxed warning: suicidality in patients <25, monitor closely early.

Don't forget

  • Fluoxetine/paroxetine raise the level, recheck at 2 weeks. Safer SSRI partners: citalopram, escitalopram, desvenlafaxine, mirtazapine, trazodone, vilazodone, vortioxetine.
  • No MAOIs. Not for bipolar depression or borderline PD.
  • Higher doses do more (unlike SSRIs), don't under-dose a fast metabolizer and call it resistance.
  • Don't stop abruptly, taper to avoid cholinergic rebound and relapse.

Desipramine asks a little more of the prescriber than an SSRI: a baseline ECG, a serum level to steer by, a disciplined hand on the quantity dispensed, and the judgment to keep it away from bipolar and borderline patients. In exchange it offers something the newer drugs often can't: potent, selective noradrenergic antidepressant action that reaches melancholic, pain-burdened, and treatment-resistant depression, delivered by the cleanest and most steerable of the tricyclics. Its one genuinely dangerous property, overdose cardiotoxicity, is not a reason to avoid the drug but a reason to prescribe it deliberately: right patient, baseline ECG, dose to the level, control the supply. Handled that way, desipramine is not a relic of the pre-SSRI era but a still-useful tool for the depressions that the modern armamentarium leaves behind.
