Why Escitalopram Matters
If you had to pick one antidepressant to start most patients on, escitalopram would be a defensible default. It is the cleanest of the SSRIs pharmacologically, one of the two best-tolerated (with sertraline), effective across the entire anxiety-depression spectrum, generic and cheap since 2012, and burdened with almost no drug-interaction baggage. In the head-to-head literature it comes out at or near the top on the combined measure that actually matters in practice: efficacy plus tolerability plus dropout.
None of that means it is a magic bullet. The honest efficacy story for every SSRI is modest: pooled against placebo, the drug-specific effect size is small (about 0.3), most of the visible improvement in trials is placebo and natural course, and only about half of patients are meaningful responders. But the responders respond well (effect size approaching 1.0 in that subgroup), the drug is safe, the downside of a failed trial is low, and the sequential trial-and-error model of depression care has to start somewhere. Escitalopram is an excellent place to start.
Escitalopram is a first-line workhorse whose value comes from its simplicity: clean CYP profile, narrow effective dose range (10-20 mg), a short list of predictable side effects, and no required labs. Master the few things that actually distinguish it (the S-enantiomer story, the citalopram QTc contrast, hyponatremia and bleeding in the elderly, and a taper that respects its above-average discontinuation syndrome) and you can use it confidently in almost any outpatient setting.
A word on the name. Escitalopram is the purified S-enantiomer of citalopram, "es-" for "S." Citalopram (Celexa) is a racemic 50/50 mix of R- and S-isomers, and essentially all the serotonergic action lives in the S-half: the S-enantiomer is roughly 40 times more potent at the serotonin transporter than the R-. There is even animal evidence that the discarded R-enantiomer actively interferes with reuptake blockade. Whether that makes escitalopram meaningfully better than citalopram or merely a cleaner, better-behaved version of the same molecule is a genuine and unsettled question, one this guide takes up directly.
Part 1: Indications: Who Is Escitalopram For?
FDA-Approved Uses
- Major depressive disorder (adults and adolescents down to age 12)
- Generalized anxiety disorder (GAD)
Escitalopram is widely and appropriately used off-label across the rest of the SSRI-responsive spectrum: panic disorder, social anxiety disorder, OCD, PTSD, PMDD, even where its specific label doesn't carry the indication. Notably, the FDA twice issued "non-approvable" letters for panic disorder despite placebo-controlled data; this is a regulatory quirk, not a signal that it doesn't work for panic. Clinically it is used for panic like any other SSRI.
The Real-World Clinical Use
Think of escitalopram less as a drug with a narrow indication and more as a broad-spectrum agent for clinically significant negative-affect states: depression, anxiety, or the enormous overlap between them. The evidence does not support carving patients into "endogenous/chemical" versus "exogenous/situational" depression for the purpose of drug selection; SSRIs help across that whole spectrum. The clinical threshold is pragmatic: enough anxiety or depression, interfering enough with life, not resolving on its own or with non-pharmacologic measures.
Where escitalopram is a particularly good first choice:
- Comorbid depression + anxiety: the SSRIs' home turf; escitalopram is well tolerated and effective for both.
- The polypharmacy patient: its clean CYP profile (below) makes it the safest SSRI to layer onto a complicated medication list.
- The patient who will need bupropion augmentation: escitalopram is the best-evidenced SSRI for combining with bupropion, precisely because it doesn't inhibit the CYP enzymes that bupropion depends on.
- The older adult, or anyone with QTc concern, currently on or considering citalopram: escitalopram is the preferred substitute (see Part 10).
Efficacy: Be Honest About the Numbers
- Across SSRIs the efficacy differences are trivial; individual SSRIs differ by only 1-2% in depression response.
- Network meta-analysis (Cipriani, Lancet 2018) placed escitalopram among the better-performing antidepressants on combined efficacy/acceptability, but any efficacy edge over its peers is "minimal at best and illusory at worst."
- Only about 5-10% of patients get a meaningful response on escitalopram that they wouldn't have gotten on citalopram at an equivalent dose.
- One reproducible signal: in pooled analyses escitalopram separated from placebo on MADRS by week 1, whereas citalopram didn't separate until week 6, though the citalopram comparator was underdosed (mean about 29 mg) and the clinical meaning of the faster separation is unclear.
- Remember publication bias: of the pre-marketing antidepressant trials (escitalopram included), only about half were actually positive, but the published record read as nearly all positive, inflating apparent effect size by roughly a third (Turner, NEJM 2008). Set expectations accordingly.
Do not oversell escitalopram as "the strongest SSRI." Its advantage over its peers is real but small and lives mostly in tolerability and a clean interaction profile, not in raw antidepressant power. Its honest pitch is: as effective as any SSRI, better tolerated than most, and the easiest to combine with other drugs. That is plenty.
Two independent datasets suggest CRP predicts SSRI response. In low-inflammation patients (CRP <1 mg/L), escitalopram monotherapy tends to work well. In higher-inflammation patients (CRP >3 mg/L), a more noradrenergic/dopaminergic agent (nortriptyline, or bupropion augmentation) outperformed the SSRI (Uher, AJP 2014; CO-MED/Jha, Psychoneuroendocrinology 2017). Not yet standard of care, but a cheap CRP in a treatment-resistant patient may earn its keep.
Part 2: Before You Start, Workup and Candidacy
Escitalopram's great practical virtue is that it requires no baseline labs and no routine drug-level monitoring. For a healthy adult you can essentially write the prescription and go. That said, a few brief checks are worth doing selectively.

Who Is a Poor Candidate?
- Anyone on an MAOI (absolute contraindication, serotonin syndrome; requires a washout, and 5 weeks off fluoxetine before starting an MAOI, though escitalopram itself needs only about 14 days)
- Known SSRI allergy
- Untreated bipolar I without mood-stabilizer coverage (relative: treat the substrate first)
- High QTc risk without cardiology input (relative: escitalopram is lower-risk than citalopram but not risk-free)
Part 3: How to Start and Dose
Formulations
Escitalopram is refreshingly simple: oral tablets in 5, 10, and 20 mg (a liquid solution also exists for fine titration or pill-averse patients). It is marketed exclusively as the oxalate salt. No extended-release version, no formulation gymnastics.
Starting Dose and Titration
- Start: 5-10 mg once daily. A common, gentle approach is 5 mg for about 1 week, then 10 mg. The 5 mg lead-in blunts the first-week GI side effects and is especially worth it in anxious patients (who are exquisitely sensitive to early activation) and the elderly.
- Target: 10-20 mg/day for most patients. Ten milligrams is a fully therapeutic dose for many.
- Titrate in 5 mg steps, roughly every 1-2 weeks as tolerated and as response dictates.
- FDA maximum: 20 mg/day.
The effective window is narrow and low: most of the therapeutic action is captured between 10 and 20 mg. This is part of what makes escitalopram easy: there is not much room to get lost.
Timing
Once daily, morning or evening, patient preference. If it's activating or causes insomnia, dose in the morning; if it's sedating or causes GI upset, dosing with food and/or in the evening can help.
The Dose Cap Nuance (This Is About Citalopram, Not Escitalopram)
Here is a point that trips people up. The hard FDA dose caps everyone remembers (at or below 20 mg in age over 60, in CYP2C19 poor metabolizers, and with strong CYP2C19 inhibitors) are a citalopram story driven by QTc. Escitalopram is metabolized by CYP2C19 too, so strong 2C19 inhibitors (omeprazole, cimetidine) do raise escitalopram levels, and it is prudent not to push above 20 mg in those settings. But escitalopram carries no FDA QTc warning and no mandated elderly dose cap the way citalopram does. Its ceiling is simply the FDA-approved 20 mg/day.
The "don't exceed 20 mg in the elderly" reflex is correct for citalopram for cardiac reasons. For escitalopram, 20 mg is simply the top of the approved range, there's no separate QTc-driven elderly restriction. This distinction is exactly why we switch older, QTc-vulnerable patients from citalopram to escitalopram.
About Doses Above 20 mg
Some clinicians use an unofficial 30 mg ceiling (occasionally in OCD), and there's scattered empirical hand-waving about efficacy at supratherapeutic doses. Ignore it. Patient-satisfaction data actually peak at the FDA-approved range, higher doses mostly buy more side effects (and more QTc), and above 20 mg is not standard practice. Don't go there without a specific reason.
The Response Timeline: Set Expectations on Day One
- The transporter is blocked instantly, but the mood effect takes about 4 weeks (up to 8-12 for full effect).
- Tell patients plainly: "You won't feel it for about a month. The first week you may feel the side effects before you feel any benefit, that's normal and it passes."
- Don't judge efficacy before 4 weeks at a therapeutic dose. If the patient is even slightly better at one month, that justifies pushing the dose up a step.
Part 4: Monitoring, What to Watch and When
There are no mandatory labs and no drug levels. Monitoring is clinical, plus a couple of targeted checks in vulnerable patients.

Targeted Monitoring in Specific Patients
- Serum sodium in older/frail patients: hyponatremia (SIADH) most often appears within the first ~14 days. Check if symptomatic (dizziness, fatigue, headache, confusion) or proactively in high-risk elders.
- ECG only if QTc risk factors exist or a QT-prolonging drug is co-prescribed. Routine EKGs are not required for escitalopram; real-world arrhythmia is rare.
The black-box suicidality warning is about the first few weeks, not the whole treatment course, and the paradox is real: untreated depression is itself the larger suicide risk. Don't withhold the drug, front-load the contact. A check-in call or visit in week 1-2, plus explicit patient/family education on agitation, worsening mood, or new suicidal thoughts, is the actual mitigation.
Part 5: Side Effects and How to Manage Them
The governing principle mirrors the whole SSRI class: early side effects are transient and dose-related; the durable ones (sexual dysfunction, emotional blunting, weight) are the real adherence enemies. Most are managed by dose reduction, timing, or a switch, not by pushing through.
Early, Transient (First 3-7 Days)
Nausea, stomachache, diarrhea, headache. These usually resolve within a week as the body adapts.
Management:
- Reassure and wait, most settle by day 7.
- Take with food.
- Slow the titration (5 mg start, longer intervals) for the sensitive patient.
- Nausea is dose-dependent, more pronounced above 10 mg, another argument for the low-and-slow start. Short-course antiemetic PRN if severe.
- Note that diarrhea is a recognized escitalopram signal (odds ratio about 1.9); if it's the dealbreaker, this is a reason to consider a different SSRI rather than escalate.
Sexual Dysfunction (the Most Common Durable Effect)
Decreased libido, delayed or absent orgasm, erectile difficulty, decreased genital sensation. Prevalence across SSRIs runs 50-70%, highly individual. Escitalopram sits in the middle of the SSRI pack: better than paroxetine, and by older data better than fluoxetine, though some newer analyses put it worse than fluoxetine.
Management, in order:
- Wait a few weeks, some adaptation occurs.
- Lower the dose, often helps and frequently preserves antidepressant benefit.
- Switch agents. Not every drug hits the same patient the same way. Bupropion and vortioxetine carry the lowest sexual burden (vortioxetine NNT is about 12 to avoid sexual dysfunction versus escitalopram, per Clayton 2015, at the cost of more nausea and higher price).
- The stopping rule: if two SSRIs cause the same sexual side effect in a patient, a third SSRI is unlikely to help, move to a non-SSRI (bupropion, mirtazapine).
Emotional Blunting / Flattening
At therapeutic doses SSRIs preferentially blunt negative emotion (that's the point). But at higher doses or in susceptible patients, all emotion goes flat, patients describe "gray," "washed out," "zombie," "robot," loss of joy.
Management:
- This is not the therapeutic target, it signals the dose is too high. Lower it.
- A minority of patients cannot find any dose that helps the depression without blunting; for them, an SSRI may not be the right class.
Gastrointestinal (Persistent)
Beyond the early phase, diarrhea is escitalopram's more characteristic ongoing GI effect (constipation is not a notable issue for it). Manage with dose reduction, timing, or switch.
Weight
Roughly 25% of SSRI patients gain weight over time; escitalopram is a moderate offender (fluoxetine is the leanest SSRI). Some of the "gain" is recovery of appetite from a depression that had suppressed it.
Management: counsel early, diet/exercise, and if weight gain is significant and unacceptable, consider a lower-weight agent (fluoxetine, bupropion).
Fatigue, Sedation, Insomnia
Variable and often self-limited. Move the dose to bedtime if sedating, to morning if activating/insomnogenic. Cognitive fog is not a prominent escitalopram problem, that reputation belongs to paroxetine, which is worth avoiding for exactly that reason.
The Elderly Triad: Hyponatremia, Bleeding, Falls
These are the side effects that turn escitalopram from "benign" to "requires attention" in older adults.
- Hyponatremia (SIADH): highest risk in the elderly, usually within 14 days. Dizziness, fatigue, headache, confusion, seizures at the extreme. Check sodium early in older patients; bupropion, mirtazapine, and TCAs carry lower risk if this is a recurring problem.
- GI bleeding: SSRIs deplete platelet serotonin and impair aggregation, roughly 1.5 times baseline upper-GI-bleed risk, rising toward about 3 times with concurrent NSAIDs, corticosteroids, or a prior bleed. A PPI substantially neutralizes the risk.
- Falls and fractures: SSRIs reduce bone density and raise fall risk (30-40% in elders), compounded by any hyponatremia/orthostasis. Discuss explicitly in osteoporotic or fall-prone patients.
In a healthy young adult the bleeding risk barely merits a sentence. In an older adult on an NSAID or anticoagulant, it's a real conversation, and the cleanest fix is often a PPI or switching the analgesic to acetaminophen, not abandoning the SSRI.
Part 6: Overdose, Toxicity, and Boxed Warnings
Escitalopram is, on the whole, a safe-in-overdose drug relative to older antidepressants, one of the reasons the SSRIs displaced TCAs. But three safety issues deserve explicit attention.
Boxed Warning: Suicidality in Youth
Like all antidepressants, escitalopram carries the FDA black-box warning for increased suicidal thinking and behavior in patients under 25, concentrated in the first weeks of treatment and after dose changes. Paroxetine and venlafaxine signal higher than escitalopram in some analyses; escitalopram itself is not a standout offender. The mitigation is close early monitoring and education, not avoidance: untreated depression is the bigger killer.
Serotonin Syndrome
Rare with an SSRI alone; the danger is combinations. Watch out for MAOIs (absolute contraindication), tramadol, linezolid, triptans, St. John's Wort, other serotonergic antidepressants, and methylene blue. Presentation: agitation/confusion, autonomic instability (tachycardia, hypertension, hyperthermia, diaphoresis), and neuromuscular signs (hyperreflexia, clonus, rigidity, tremor). Management is stopping the serotonergic agents and supportive care; usually resolves quickly if caught early, occasionally life-threatening.
The most common real-world serotonin-syndrome trap is not an exotic MAOI, it's tramadol for pain (or linezolid as an antibiotic) started by another prescriber in a patient already on the SSRI. Flag these two by name in the chart and to the patient.
QTc Prolongation: The Escitalopram-vs-Citalopram Story
This is escitalopram's signature safety talking point, and it's mostly reassuring.
- Escitalopram prolongs QTc dose-dependently, and ranks second among SSRIs behind citalopram, but at roughly half citalopram's magnitude at equivalent doses: escitalopram 30 mg is about 10.7 ms QTc prolongation, versus citalopram 60 mg at about 18.5 ms.
- Escitalopram carries no FDA QTc warning (citalopram does).
- Two large population studies found higher rates of cardiac arrest and serious arrhythmia with citalopram than escitalopram (Qirjazi, PLoS One 2016; Weeke, Clin Pharmacol Ther 2012).
- The cautionary counterpoint, the VA "citalopram lowering" experience (Rector, AJP 2016; 35,848 veterans): forcing citalopram doses down into the "safe" QTc range produced a sharp rise in all-cause hospitalizations and deaths with no drop in arrhythmias. The lesson isn't that QTc doesn't matter, it's that undertreating depression to chase a small QTc number can harm more patients than the arrhythmia would. The smarter move for a QTc-vulnerable patient is often to switch to escitalopram, not to cripple the citalopram dose.
Practically: routine ECGs are unnecessary. Get one, and use caution, when baseline QTc is prolonged, in significant cardiac disease, with electrolyte derangement, or when stacking other QT-prolonging drugs.
Part 7: Drug Interactions
This is escitalopram's quiet superpower. It is the cleanest SSRI for drug-drug interactions (alongside citalopram and sertraline), because it neither induces nor inhibits CYP enzymes: no clinically meaningful CYP2D6 inhibition, no CYP2C19 inhibition.
Contrast that with the SSRIs you have to think harder about:
- Fluoxetine and paroxetine: potent CYP2D6 inhibitors (raise TCAs, some antipsychotics, certain beta-blockers/antiarrhythmics; can blunt tamoxifen).
- Fluvoxamine: strong CYP1A2/3A4 inhibitor, wide interaction footprint.
- Escitalopram: essentially stays in its lane.
What Still Matters for Escitalopram

The Bupropion Advantage
Because escitalopram doesn't touch CYP2D6/2C19, it is the best-evidenced SSRI to augment with bupropion. Other SSRIs (especially paroxetine/fluoxetine) inhibit the enzymes bupropion is cleared by, raising bupropion levels and seizure risk. If your plan for a partial responder involves adding bupropion, starting from escitalopram is the smart setup.
When a patient is on a complicated medical regimen (anticoagulants, antiarrhythmics, transplant drugs, HIV meds, chemo), escitalopram (or sertraline) should be near the top of your list precisely because it does nothing interesting to the CYP system. In polypharmacy, boring is a feature.
Part 8: Special Populations
Pregnancy
SSRIs cross the placenta, and the decision is always a benefit-risk balance against the substantial harms of untreated perinatal depression. Within the class:
- Sertraline is the preferred SSRI in pregnancy and lactation (most reassuring data). Paroxetine is the one to avoid (a first-trimester cardiac-malformation signal; category D).
- Escitalopram is less studied than sertraline but does not carry paroxetine's baggage; it's a reasonable choice, particularly if the patient is already stable on it: switching a well-controlled patient to sertraline purely on principle risks destabilizing her.
- Third-trimester exposure can cause a self-limited neonatal adaptation/withdrawal syndrome (jitteriness, tremor, feeding difficulty) in a minority of newborns; usually resolves without intervention. Coordinate with OB and pediatrics.
The governing principle: for moderate-to-severe illness, continuing an effective antidepressant usually beats the relapse risk of stopping. Individualize, document the conversation, and loop in obstetrics.
Lactation
Sertraline and paroxetine have the lowest milk transfer and are best studied; escitalopram is less characterized but generally considered compatible with breastfeeding with routine infant observation. If the mother is already doing well on escitalopram, continuing it is often the right call, coordinate with pediatrics.
Elderly
Escitalopram is a preferred SSRI in older adults, largely because it's the safer half of the citalopram molecule: no FDA QTc dose cap, lower QTc than citalopram, and the lowest interaction risk of the class in patients who are usually on many other drugs. It has a solid evidence base in late-life depression (alongside sertraline and citalopram) and is effective for behavioral/psychological symptoms of dementia.
Practical geriatric dosing:
- Start low: 5 mg, advance to 10 mg after a week; 20 mg only as needed.
- Check serum sodium within about 14 days (hyponatremia risk peaks early).
- Screen bleeding and fall/fracture risk; mind NSAIDs and anticoagulants.
- Expect somewhat lower remission rates than in younger adults with the same drug.
Pediatric and Adolescent
- FDA-approved from age 12 for depression and from age 7 for GAD, but the pediatric depression trials showed mixed, modest benefit.
- Fluoxetine is first-line in childhood depression (approved down to age 8, strongest evidence); escitalopram, citalopram, and sertraline are options with weaker data.
- The black-box suicidality warning is most pointed here, monitor closely in the first weeks. Reassuringly, escitalopram is not the pediatric agent most linked to suicidality (that flag belongs to paroxetine).
Renal and Hepatic Impairment
- Renal: SSRIs are largely non-renally cleared; escitalopram is generally safe even in significant renal impairment, with no routine dose change. Use some caution in end-stage disease.
- Hepatic: escitalopram is cleared hepatically (CYP2C19). In significant liver disease, use a lower dose and/or slower titration.
Bipolar Disorder
Escitalopram's antidepressant-induced switch rate to hypomania is relatively low (3-8%), and there's thin pilot evidence (n=10) that 10 mg/day might even behave like a mild mood stabilizer in bipolar II. Do not use it as monotherapy in bipolar depression without mood-stabilizer cover, screen for bipolarity before starting, and monitor mood if you do use it in a bipolar-spectrum patient.
Part 9: Discontinuation: Taper It, and Mean It
Escitalopram's discontinuation syndrome is above average for the class, second only to paroxetine, and clearly higher than the long-half-life fluoxetine. This is the one place its otherwise easygoing profile bites.
What withdrawal looks like: dizziness, nausea, fatigue, muscle aches, chills, irritability, insomnia, flu-like malaise, and the classic "brain zaps" (brief, non-painful electric-shock sensations in the head), plus rebound anxiety/depression. Across the class, roughly 60% have no symptoms, about 35% mild-to-moderate, about 5% severe.
Two things to tell every patient up front:
- This is uncomfortable, not dangerous. Unlike benzodiazepine or alcohol withdrawal, SSRI discontinuation won't hurt you, but it can feel bad enough to scare people into thinking they're "addicted," which then poisons their willingness to ever take psychiatric medication again. Pre-empt that fear.
- Never stop abruptly. We taper, and if it gets rough, we slow down.
How to stop:
- Taper gradually. There's no single mandated schedule, stepping down (e.g., 20 to 10 to 5 mg, with the liquid available for smaller steps near the end) over weeks is typical, slower for long-term or high-dose users.
- The iterative rule: if withdrawal symptoms emerge at a given step, go back to the last comfortable dose and taper more slowly. Done patiently, this essentially always succeeds.
- Short courses (a few months) generally taper easily; long-term, high-dose use warrants a slower, hyperbolic taper near the bottom.
- If a patient needs to come off fast and can't tolerate the taper, one class-level trick is to bridge to fluoxetine (long half-life equals built-in taper) and stop that.
When to stop at all:
- First episode of depression: continue about 6 months past remission, then attempt a slow taper; if symptoms return, resume and try again later.
- Recurrent or chronic conditions (recurrent MDD, GAD, OCD, dysthymia): many patients stay on indefinitely, withdrawal is a non-issue as long as they keep taking it, and the relapse cost of stopping is real.
- Situational depression: match the drug's course to the life course.
Escitalopram's discontinuation profile is the single most important way it departs from the "just stop it" reputation SSRIs sometimes carry. Bring up the taper at the first prescription, not at the last. A patient who was warned in advance treats brain zaps as an expected, manageable nuisance; a patient who wasn't treats them as proof the drug damaged them.
Part 10: Escitalopram vs the Alternatives
vs Citalopram (Its Own Parent)
The central comparison, and a genuinely close call.
- Efficacy: essentially equal. Escitalopram may hold a whisker's edge in pooled/head-to-head data, but only 5-10% of patients meaningfully benefit from escitalopram who wouldn't from equivalent citalopram.
- Cardiac safety: escitalopram wins, roughly half the QTc prolongation at equivalent doses, no FDA QTc warning, and lower real-world arrhythmia rates.
- Dose caps: citalopram is capped (40 mg general; 20 mg in age over 60, CYP2C19 poor metabolizers, or strong-inhibitor co-use); escitalopram has no such QTc-driven cap.
- Conversion: roughly halve the citalopram dose (citalopram 40 to escitalopram 20; 20 to escitalopram 10). When switching, cross-taper gently over 2-4 weeks rather than swapping overnight.
- Bottom line: for a new start they're near-interchangeable; for an older or QTc-vulnerable patient, or one on a strong 2C19 inhibitor, escitalopram is the better half of the molecule.
vs the Other SSRIs
- Sertraline: the other "good all-around" SSRI and escitalopram's main peer. Sertraline edges ahead in pregnancy, lactation, and cardiac disease (best cardiac-safety data). Reasonable to consider them co-first-line and let patient specifics decide.
- Fluoxetine: first-line in kids, the least withdrawal (long half-life), and the least weight gain, but a potent CYP2D6/2C19 inhibitor, so it loses to escitalopram on interactions.
- Paroxetine: the problem child: worst sexual dysfunction, worst withdrawal, most weight gain, sedation, anticholinergic load, and its own QTc. Avoid it; escitalopram beats it on essentially every axis.
- Fluvoxamine: strong CYP1A2/3A4 inhibitor, US-approved only for OCD, a niche agent, not a first-line comparator.
vs Non-SSRI Antidepressants
- SNRIs (venlafaxine, duloxetine): marginally more effective on average, but the difference reaches only about 1 in 24 patients, and they bring more side effects, more withdrawal (venlafaxine especially), and, for venlafaxine, a blood-pressure signal. Not clearly superior; escitalopram is the gentler first move.
- Bupropion: roughly equal antidepressant efficacy, even with anxious features, and superior on sexual function, weight, and cognition, but weaker for prominent anxiety, where SSRIs are better. Also the ideal augmentation partner for escitalopram.
- Vortioxetine: similar efficacy, fewer sexual side effects (NNT about 12 versus escitalopram to avoid them), but more nausea and higher cost.
- Mirtazapine: slightly more effective than SSRIs in meta-analysis (again about 1 in 24), useful when you want sedation and appetite, at a metabolic cost.
Where Escitalopram Sits
Escitalopram earns its default-first-line status not by being the strongest antidepressant, but by being the best-balanced SSRI: top-tier tolerability, the cleanest interaction profile in the class, a simple 10-20 mg dose range, no required labs, the safer half of the citalopram molecule, and the best partner for bupropion augmentation. Its two real caveats are a moderate sexual-dysfunction rate and an above-average discontinuation syndrome, both manageable if you plan for them.
Mechanism: The Short Version
Escitalopram is the purified S-enantiomer of citalopram, and it is a highly selective serotonin-transporter (SERT) blocker with minimal off-target binding. The S-isomer is roughly 40 times more potent at SERT than the R-isomer (which citalopram carries as inert, and possibly counterproductive, ballast). Blocking SERT raises synaptic serotonin immediately, yet the antidepressant effect takes about a month, the familiar SSRI paradox. The leading explanations: it takes weeks to desensitize the serotonin 1A autoreceptors that initially damp the signal, and downstream BDNF upregulation leading to synaptogenesis/neuroplasticity builds over the same timescale. In plain terms, the drug flips a switch on day one but the brain takes weeks to remodel around it. The clean, high selectivity of the S-enantiomer is also the pharmacologic root of escitalopram's two nicest practical features: minimal CYP interference and lower QTc than the racemic parent.
The Bedside Cheat Sheet
Starting
- Start 5 mg for about 1 week, then 10 mg. Target 10-20 mg/day. FDA max 20 mg
- Titrate 5 mg every 1-2 weeks. Tablets 5/10/20 mg (liquid available)
- No required baseline labs. Screen for bipolarity; ECG only if QTc risk

Dose caps
- The elderly/CYP2C19 20 mg cap is a citalopram rule (QTc). Escitalopram has no QTc dose cap, 20 mg is just the approved ceiling
- Keep at or below 20 mg with strong CYP2C19 inhibitors (omeprazole, cimetidine)
Side effects
- Early (GI, headache): transient, with food, slow titration
- Sexual dysfunction (50-70%): lower dose, then switch, then non-SSRI (bupropion). Two SSRIs, same effect, stop trying SSRIs
- Emotional blunting equals dose too high. Lower it
- Elderly triad: hyponatremia (check sodium in first ~14 days), GI bleed (1.5-3x with NSAIDs), falls/fractures
Safety
- Black box: suicidality under 25, first weeks, monitor early, don't avoid
- Serotonin syndrome: MAOIs (absolute contraindication), tramadol, linezolid, triptans, methylene blue
- QTc: lower than citalopram (about half), no FDA warning; routine ECG not needed
Interactions
- Cleanest SSRI, no CYP induction/inhibition. Ideal in polypharmacy
- Best SSRI for bupropion augmentation
- Raised by strong CYP2C19 inhibitors; increased bleeding with NSAIDs/anticoagulants
Don't forget
- Taper it, discontinuation syndrome is 2nd worst after paroxetine. Warn about brain zaps on day one; go back a step and slow down if withdrawal hits. Uncomfortable, not dangerous
- In the elderly / QTc-vulnerable, escitalopram is the preferred substitute for citalopram
- Sertraline is the better choice in pregnancy/lactation and cardiac disease
Escitalopram won't do anything dramatic for the literature, its efficacy edge over its SSRI cousins is small enough to argue about. What it does instead is remove friction: no labs, a two-number dose range, the cleanest interaction profile in psychiatry's most-prescribed class, the safer half of the citalopram molecule, and an easy on-ramp to bupropion when you need more. Respect its two rough edges, a real sexual-side-effect rate and a real discontinuation syndrome, and you have a first-line antidepressant you can hand to almost anyone, from a medically complicated 80-year-old to an anxious teenager, with confidence. In a field where the hard part is usually starting, that reliability is worth a great deal.
