Why Fluvoxamine Still Has a Place

Fluvoxamine is the SSRI that most American prescribers have never actually started. It was the first SSRI approved for OCD in the United States, it is a workhorse for obsessive-compulsive disorder in much of the world, and it carries a genuinely useful pharmacologic quirk (potent, broad cytochrome P450 inhibition) that occasionally makes it the right drug rather than merely an available one. Yet it sits near the bottom of most clinicians' SSRI hierarchies, and for defensible reasons: it is a promiscuous enzyme inhibitor that complicates polypharmacy, it produces more GI upset than its peers, and it is among the worst SSRIs for discontinuation syndrome.

So this is not a first-line antidepressant, and this guide will not pretend otherwise. But fluvoxamine is a precise instrument for a few specific jobs.

The thesis of this guide

Fluvoxamine is a legitimate choice for OCD and for the anxiety-spectrum patient on minimal other medications, and it becomes a liability the moment the medication list gets long. Know its enzyme profile cold, respect its discontinuation, and you have a serviceable, inexpensive tool with the lowest sexual-side-effect burden of any SSRI.

A note on where it fits: for straightforward depression, escitalopram or sertraline will almost always be the better opening move, with cleaner interactions, an easier taper, and a gentler stomach. Reach for fluvoxamine when the indication is OCD, when a patient has responded to it before, or when you actually want its CYP1A2 inhibition working for you (the clozapine trick below). Everywhere else, it is a second- or third-string agent.


Part 1: Indications: Who Is Fluvoxamine For?

FDA-Approved Uses

  • Obsessive-compulsive disorder (OCD): the flagship indication, in adults and in children/adolescents ages 8 and up (the immediate-release form carries a pediatric OCD indication)
  • Major depressive disorder (approved in many markets; in the U.S. its identity is dominated by OCD, but it is a fully effective antidepressant)

The controlled-release capsule (Luvox CR) additionally carries a social anxiety disorder indication.

OCD: The Home Turf

This is where fluvoxamine earns its keep. It is a well-established, effective SSRI for obsessions and compulsions, with the same core caveats that apply to every serotonergic drug in OCD:

  • OCD frequently needs higher doses than depression, often toward the top of the range, 200 to 300 mg/day.
  • OCD responds slowly: allow 8 to 12 weeks at an adequate dose before calling a trial a failure, versus 4 to 6 weeks in depression.
  • Response is usually partial (a meaningful reduction in symptoms), not remission. A 25 to 35% Y-BOCS reduction is a real, worthwhile response in OCD, and worth holding onto.
  • Pair it with exposure and response prevention (ERP). Medication plus ERP beats either alone; the drug lowers the temperature enough to make the therapy possible.
Pearl

Don't abandon fluvoxamine in OCD at week 6 because the patient is only a little better. OCD is the one indication where you titrate high and wait long. Push toward 200 to 300 mg/day and give it the full 10 to 12 weeks before you switch or augment. Many "fluvoxamine failures" were simply under-dosed or under-timed.

Major Depression

Effective, but unremarkable relative to the class, and burdened by more GI upset and a harder taper than escitalopram or sertraline. In the head-to-head efficacy literature (the Cipriani network meta-analysis of 21 antidepressants), fluvoxamine sits in the middle of the pack on efficacy and toward the lower end on acceptability. There is rarely a positive reason to choose it for uncomplicated depression.

Anxiety-Spectrum Disorders

Fluvoxamine has real efficacy across the anxiety disorders: panic disorder, social anxiety disorder (the CR form is FDA-approved here), generalized anxiety, and PTSD are all reasonable off-label or on-label targets. In the panic-disorder network meta-analyses, fluvoxamine shows favorable efficacy but a higher adverse-effect burden than sertraline or escitalopram, which is the recurring theme of this drug: it works, but its peers are more comfortable to take.

The Schizo-OCD Niche

OCD symptoms are common in schizophrenia, and fluvoxamine is effective for them, but here its enzyme inhibition becomes genuinely dangerous, raising antipsychotic levels 2- to 10-fold. See the interactions and special-populations sections; this is a place where escitalopram is often the safer OCD-directed choice despite lacking an OCD indication.

The Clozapine Maneuver

Where fluvoxamine's biggest liability becomes an asset: fluvoxamine's potent CYP1A2 inhibition can be deployed on purpose. See Part 7; it is one of the few times you reach for this drug because of its interactions rather than in spite of them.

Who Is a Good Candidate

  • Patients with OCD, especially those who have responded to fluvoxamine before or failed a cleaner SSRI
  • Patients on minimal concurrent medication (the fewer CYP substrates on board, the safer fluvoxamine gets)
  • Patients for whom sexual dysfunction is a dealbreaker: fluvoxamine has the lowest rate of sexual side effects among SSRIs, a real and underused advantage
  • Patients who need or want an SSRI and have a specific reason to avoid the alternatives

Who Is a Poor Candidate

  • Anyone on complex polypharmacy, particularly with CYP1A2, CYP3A4, or CYP2C19 substrates on board (theophylline, tizanidine, clozapine, warfarin, many benzodiazepines): fluvoxamine is on the short list of antidepressants with the greatest potential for problematic drug interactions, alongside the MAOIs and paroxetine
  • Smokers on clozapine or theophylline where enzyme dynamics are already unstable
  • Patients with a history of severe SSRI discontinuation syndrome, or who are likely to stop abruptly or run out of medication
  • Uncomplicated depression where a cleaner SSRI will do the same job more comfortably

The single best predictor of whether fluvoxamine is a good idea is the length of the patient's medication list. On a monotherapy patient it behaves like an ordinary SSRI. On a medically complex patient with five other drugs, it is a minefield of 2- to 10-fold level shifts. Count the substrates before you write it.


Part 2: Before You Start: Workup

Fluvoxamine requires no routine laboratory monitoring, no ECG, and no drug levels for monotherapy in an otherwise healthy patient. It is an SSRI, not lithium. The workup is a medication reconciliation and a conversation, not a lab panel.

Before You Write It

  • Reconcile every medication and supplement, prescription and OTC. This is the non-negotiable step. Specifically look for CYP1A2 substrates (clozapine, olanzapine, theophylline, tizanidine, caffeine, duloxetine, ramelteon, agomelatine), CYP3A4 substrates (many benzodiazepines, buspirone, guanfacine, statins, some antipsychotics), CYP2C19 substrates, and warfarin. If the list is long, reconsider the drug.
  • Screen for bipolarity before any antidepressant, since an SSRI can flip a susceptible patient into mania or mixed states.
  • Ask about NSAIDs and anticoagulants: the SSRI-class GI-bleeding interaction applies (see below).
  • In women of childbearing potential, take a reproductive history and discuss pregnancy planning.
  • Set expectations: about a month to benefit in depression, longer in OCD; early GI side effects that usually fade; and, critically, the plan to never stop abruptly.

If you are adding fluvoxamine to clozapine or another CYP1A2-dependent drug, get a baseline level of that drug before you start (see Part 7). That is the one "lab" fluvoxamine sometimes demands, and it is about the other drug, not fluvoxamine.


Part 3: How to Start and Dose

Formulations

  • Immediate-release tablets: 25, 50, and 100 mg. The generic, inexpensive workhorse. Carries the pediatric OCD indication.
  • Controlled-release capsules (Luvox CR): 100 and 150 mg. Once-daily, smoother, FDA-approved for OCD and social anxiety disorder; more expensive and often less available.

Immediate-release fluvoxamine has a short half-life (about 15 to 20 hours), notably shorter than most SSRIs, which is part of why it is dosed twice daily at higher totals and why it discontinues so harshly. The CR formulation allows once-daily dosing.

  • Start 50 mg at bedtime for adults. The short half-life and the bedtime timing both help with the early nausea and sedation.
  • Titrate by 50 mg every 4 to 7 days as tolerated, toward the therapeutic range.
  • Once you exceed about 100 mg/day, split the dose twice daily, because the short half-life makes once-daily dosing at high totals uneven. A common structure gives the larger portion at bedtime.
  • Usual therapeutic range: 100 to 300 mg/day. The maximum is 300 mg/day.
  • Depression often responds in the 100 to 200 mg/day range; OCD frequently needs 200 to 300 mg/day.
Pearl

Fluvoxamine's short half-life is a double-edged sword. It clears fast enough that a missed dose or two can trigger discontinuation symptoms within the ongoing course of treatment, and patients feel "brain zaps" on the day they forget a dose. Warn them up front, and once you're above 100 mg, dose it twice daily so troughs don't crater.

The Dose-Response Reality

Fluvoxamine was conspicuously excluded from the major SSRI dose-response meta-analyses, so its optimal-dose curve is less well characterized than sertraline's or escitalopram's. Apply the general SSRI principle with a modification for OCD: for depression, the class lesson holds, benefits plateau and side effects climb past moderate doses, so don't reflexively push to the ceiling. For OCD, the higher end of the range is genuinely where much of the benefit lives, so titrate up more willingly and give it time.


Part 4: Monitoring

For monotherapy in a healthy patient, there is essentially nothing to draw. The monitoring here is clinical and interaction-driven, not laboratory.

  • Weeks 1 to 4: check in for early GI side effects (usually resolve by day 3 to 7), activation/insomnia, and, especially in patients under 25, suicidality (the SSRI black-box warning applies; risk is highest in the first 2 weeks and after dose changes).
  • About 4 weeks: assess early response in depression/anxiety; even "slightly better" justifies continuing and titrating. In OCD, don't judge yet; give it to 8 to 12 weeks.
  • Elderly / first 2 weeks: consider a sodium check if the patient is symptomatic (lethargy, confusion, nausea); SSRI-induced SIADH/hyponatremia is most common early and in older patients.
  • Ongoing: track sexual function, weight, emotional blunting, and GI tolerability at routine visits.

The one situation that demands real monitoring is co-prescription with a narrow-therapeutic-index CYP substrate, above all clozapine. There, check the substrate's level before and after adding fluvoxamine, and follow it regularly. That is a monitoring obligation for the other drug that fluvoxamine imposes.


Part 5: Side Effects and How to Manage Them

Fluvoxamine's side-effect profile is the ordinary SSRI menu, with two distinguishing features: more GI upset than average and less sexual dysfunction than average. The governing principle is the same as for any SSRI: most early effects are transient and dose-related; manage them so the patient stays on long enough to benefit.

Gastrointestinal: The Standout Burden

Fluvoxamine is among the top SSRIs for diarrhea (odds ratio about 2.3 in meta-analysis, in the same tier as sertraline), and nausea is common early.

  • Start low, titrate slowly, and take with food (the single most effective nausea measure).
  • Dose at bedtime so the peak passes during sleep.
  • Nausea is usually an early, self-limiting effect (days to a couple of weeks); tell patients that in advance so they don't quit on day 3.
  • For persistent nausea, ondansetron 4 mg PRN is a clean antiemetic (no tardive-dyskinesia risk); ginger is a benign OTC option.
  • For diarrhea that doesn't settle, dose reduction or a switch may be needed, which is one of the more common reasons fluvoxamine gets abandoned.

Sexual Dysfunction: Fluvoxamine's Relative Advantage

SSRIs as a class cause sexual dysfunction in 50 to 70% of patients (decreased libido, delayed or absent orgasm, reduced genital sensation). Fluvoxamine has the lowest reported rate among the SSRIs: still a class effect, but comparatively favorable. This is a legitimate reason to choose fluvoxamine for a patient in whom sexual side effects are a dealbreaker and OCD/anxiety is the target.

  • Lower the dose if efficacy allows.
  • Weekend drug holidays (skip Friday/Saturday, resume Sunday) have open-label support and are feasible with fluvoxamine's short half-life, but only if the patient does not develop discontinuation symptoms across the skip and is not at risk of relapse. Given fluvoxamine's harsh discontinuation profile, many patients will feel worse from the holiday than from the side effect; use this cautiously here.
  • Switch or augment (e.g., add bupropion) if the problem persists, though if you've reached for fluvoxamine specifically for its low sexual burden, a switch may be self-defeating.

Activation, Insomnia, and Anxiety

Early jitteriness, insomnia, or a transient increase in anxiety can occur, as with any SSRI, especially early. Start low, dose at bedtime, and reassure; benzodiazepine coverage for the first week or two is reasonable in a highly anxious patient, but prefer a glucuronidated benzodiazepine (lorazepam, oxazepam, temazepam), because fluvoxamine will raise the levels of CYP-metabolized benzos like alprazolam (see Part 7).

Emotional Blunting

Blunting of both positive and negative affect ("gray," "flat," "robotic") is a dose-dependent SSRI effect, not expected at appropriate doses. If it appears, lower the dose; if the therapeutic effect can't be preserved without blunting, the drug may not be right for that patient.

Weight and Metabolic

Weight change with fluvoxamine is modest and broadly in line with the SSRI class (roughly a quarter of patients gain some weight over time; mechanisms include appetite recovery). Manage with the usual diet/activity counseling; it is not a leading reason to avoid this drug.

Other

  • Sweating, tremor, fatigue: usual mild SSRI effects; manage with timing and dose.
  • Sedation: more prominent than with activating SSRIs; leverage it by dosing at night.
  • Apathy / RLS: occasional SSRI-class effects; SSRI-induced restless legs can respond to a dopamine agonist (e.g., pramipexole) if troublesome.

Part 6: Toxicity, Bleeding, and the Boxed Warning

Overdose

Fluvoxamine, like the SSRIs generally, is relatively safe in overdose compared with TCAs, MAOIs, or lithium. Isolated SSRI overdose is rarely lethal; the dangerous scenarios are serotonin syndrome (usually from a serotonergic combination, not SSRI monotherapy) and QT/seizure effects at very large ingestions or in combination overdoses. Management is supportive and belongs in an emergency department, but the drug is not lethal in the way its narrow-window competitors are.

Serotonin Syndrome

Risk rises sharply when fluvoxamine is combined with other serotonergic agents: MAOIs (contraindicated), tramadol, linezolid, methylene blue, triptans, other antidepressants, St. John's wort. The picture is agitation, autonomic instability (tachycardia, hypertension, hyperthermia), and neuromuscular findings (hyperreflexia, clonus, rigidity, tremor). Discontinue the serotonergic agents and provide supportive care; it is usually reversible if caught early. The triptan risk, though widely feared, is very low in practice.

GI Bleeding: The SSRI-Class Warning That Applies Here

SSRIs impair platelet aggregation (platelets can't synthesize serotonin, so they depend on plasma uptake). This produces a real but modest bleeding signal.

  • Roughly 1 extra GI bleed per about 8,000 prescriptions at baseline.
  • NSAIDs multiply this 7- to 15-fold. Warfarin, clopidogrel, and antiplatelet agents also raise it.
  • A proton-pump inhibitor (e.g., omeprazole) largely neutralizes the added GI-bleeding risk.

Practically: mention it to patients with a prior GI bleed, current NSAID/anticoagulant use, or upcoming surgery; co-prescribe a PPI when risk is high; and prefer acetaminophen over NSAIDs. Note that fluvoxamine's CYP inhibition can also raise warfarin levels, compounding the bleeding risk, a double hit worth flagging.

Hyponatremia / SIADH

An SSRI-class effect, most common in the first weeks and in the elderly. Symptoms are lethargy, confusion, nausea, and, in severe cases, seizures. Check a sodium in symptomatic or elderly patients; treat with fluid restriction (typically 1 to 1.5 L/day) and, if needed, discontinuation.

Boxed warning: suicidality in patients under 25

Like all antidepressants, fluvoxamine carries the FDA black-box warning for increased suicidal ideation/behavior in patients under 25, concentrated in the first weeks of treatment and after dose changes. Monitor closely early, screen at every early contact, and educate patients and families about warning signs. The absolute risk is small, and the countervailing benefit of treating the illness is real, but the monitoring obligation is non-negotiable, especially in the OCD population, which skews young.


Part 7: Drug Interactions: The Whole Reason to Be Careful

This is the section that matters most for fluvoxamine. More than any other SSRI, fluvoxamine's identity is defined by its cytochrome P450 inhibition. It is a potent, broad inhibitor hitting a wide array of enzymes.

  • CYP1A2, potent (the signature; the basis of the clozapine effect and the caffeine/theophylline/tizanidine interactions)
  • CYP2C19, potent
  • CYP3A4, significant (with additional effects on 2C9 and others)

This breadth is why fluvoxamine is grouped with the MAOIs and paroxetine as the antidepressants with the greatest potential for problematic drug interactions. The clinical consequence is simple: fluvoxamine raises the levels of a long list of co-prescribed drugs, sometimes dramatically.

The counterintuitive pearl: clozapine, the interaction you can use

Fluvoxamine's CYP1A2 inhibition blocks conversion of clozapine to norclozapine, the metabolite most associated with clozapine's metabolic side effects (weight gain, dyslipidemia, glucose dysregulation). In practice, serum clozapine can stay comparable while norclozapine drops, potentially improving the metabolic profile. For a patient who needs clozapine but is suffering its metabolic side effects, low-dose fluvoxamine can be added deliberately to shift the clozapine/norclozapine ratio. But this is an advanced maneuver: drug interactions here are hard to predict, and clozapine has a narrow window. Check a clozapine level before and after adding fluvoxamine, follow it regularly, and go slow. Done carelessly, the same enzyme block that helps the metabolics can drive clozapine toward toxicity and seizures. This is fluvoxamine's biggest liability repurposed as its most distinctive benefit, but only in expert hands.

The Management Strategy in One Line

Before you start fluvoxamine, list every CYP1A2/2C19/3A4 substrate the patient takes, and either avoid the combination or plan to reduce and monitor that substrate. When in doubt on a polypharmacy patient, a cleaner SSRI (escitalopram, sertraline) spares you the entire problem.


Part 8: Special Populations

Pregnancy

Fluvoxamine is handled like the SSRI class. The overall SSRI picture: a low absolute teratogenic risk, a small independent signal for perinatal complications and (in third-trimester exposure) a neonatal adaptation syndrome (jitteriness, tremor, feeding difficulty, respiratory distress) in a minority of exposed newborns, usually self-limited, and a debated, small PPHN association. Against this sits the real fetal and maternal harm of untreated illness, much of it mediated behaviorally (missed prenatal care, substance use, poor nutrition).

Practical framework: individualize. For mild illness, non-pharmacologic management or a taper may be reasonable; for moderate-to-severe OCD/depression where the patient has responded specifically to fluvoxamine, continuing it is often the right call. Sertraline is the more commonly chosen SSRI in pregnancy given its extensive reproductive-safety data and low milk transfer, so if you are selecting an SSRI de novo in a pregnant patient, that is usually the better default. Coordinate with obstetrics, and watch the neonate near delivery.

Lactation

Fluvoxamine is excreted into breast milk in low amounts, and it is generally considered relatively compatible with breastfeeding, though the reproductive-safety database is smaller than for sertraline (the usual first choice in lactation). If a mother is already stable on fluvoxamine, continuing it while monitoring the infant for feeding, sedation, and irritability is reasonable.

Elderly (Age Over 65)

  • Start low (25 mg), titrate slowly, expect a lower effective dose (reduced clearance).
  • Higher hyponatremia/SIADH risk, especially in the first two weeks; check sodium if symptomatic.
  • Higher fall and bleeding risk, and more concurrent medications, which magnifies the CYP-interaction hazard. Do a careful medication reconciliation.

Children and Adolescents

  • Fluvoxamine IR is FDA-approved for OCD from age 8, one of its distinguishing features.
  • Start 25 mg, titrate by 25 mg, usual range 50 to 200 mg/day, max 200 mg/day (below the adult ceiling); divide BID above 50 mg.
  • The black-box suicidality warning is most salient here; monitor closely early, and involve caregivers.

Renal and Hepatic Impairment

  • Renal: SSRIs are minimally renally cleared; no major adjustment is generally required, though caution and slower titration are sensible in significant impairment.
  • Hepatic: fluvoxamine is hepatically metabolized; start lower and titrate more slowly in liver disease, and expect reduced clearance.

Part 9: Discontinuation: Handle With Care

One of the worst SSRIs to stop

Fluvoxamine is one of the worst SSRIs for discontinuation syndrome; it sits on the short list of the five worst offenders across all antidepressants (with venlafaxine, desvenlafaxine, duloxetine, and paroxetine). Its short half-life is precisely why: levels fall fast, giving the brain no built-in taper. This is a core reason it should rarely be first-line.

What discontinuation looks like: dizziness, "brain zaps" (brief electric-shock sensations), nausea, flu-like malaise, irritability, insomnia, and rebound anxiety, typically within a day or two of a missed dose or an abrupt stop, because the drug clears so quickly.

How to Stop

  • Taper slowly, slower than you would for sertraline or escitalopram. There is no fixed maximum-safe rate, but plan on weeks to months, not days, particularly after long or high-dose use.
  • Go hyperbolic near the end: the smallest doses matter most; the last steps down should be the slowest and smallest. Use the 25 mg and 50 mg tablets (or a compounded liquid) to make fine reductions.
  • If withdrawal emerges, slow down or step back up to the last tolerated dose and re-taper more gradually. This is expected and manageable; it is not a failure.
  • A pragmatic bridge for a truly stuck taper is to cross over to fluoxetine (long half-life, self-tapering) and then discontinue that, a standard trick for the difficult short-half-life SSRIs.
  • Educate on day one: never stop abruptly, don't run out, and call before discontinuing. Because fluvoxamine punishes even a single missed dose, adherence counseling is discontinuation prevention.
Pearl

Warn the patient at the start that this particular SSRI does not tolerate being forgotten. "If you miss a day, you may feel dizzy or get little electric-zap sensations, that's the medicine leaving your system fast, not a new problem. Don't let the bottle run empty, and never stop on your own." Framing it early turns a scary experience into an expected, manageable one.

When to consider stopping at all: for episodic depression, consider a taper after about 6 to 12 months of sustained wellness; for OCD, which is typically chronic and relapsing, long-term continuation is often appropriate, and if the patient is well, there is frequently no reason to invite a difficult taper at all.


Part 10: Fluvoxamine vs the Alternatives

  • vs Escitalopram / Sertraline (the preferred SSRIs): these "rise to the top" for a reason: cleaner interaction profiles, gentler discontinuation, and (for sertraline/escitalopram) strong tolerability. For uncomplicated depression or anxiety, choose one of them over fluvoxamine almost every time. Fluvoxamine wins only when its specific advantages apply: OCD, lowest sexual dysfunction, prior response, or a deliberate use of its CYP1A2 inhibition.
  • vs Paroxetine: both are interaction-heavy and hard to discontinue, but paroxetine adds the worst sexual-dysfunction and anticholinergic/cognitive burden of the class. Between two problematic SSRIs, fluvoxamine at least offers low sexual side effects and a legitimate OCD pedigree. Neither should be a casual first pick.
  • vs Fluoxetine: fluoxetine's long half-life makes it far easier to discontinue and forgiving of missed doses, the opposite of fluvoxamine. Fluoxetine is also a strong OCD agent. Fluvoxamine's edges are lower sexual dysfunction and its 1A2 inhibition; fluoxetine's are the smooth taper and a huge evidence base.
  • vs Clomipramine / SNRIs in OCD: clomipramine is arguably the most potent single agent in OCD but carries TCA toxicity and side effects; SSRIs (including fluvoxamine) are safer first-line, with clomipramine held in reserve. Among SSRIs for OCD, fluvoxamine is a perfectly respectable choice: the class is roughly comparable, so tolerability and interactions decide.
Bottom line

Where fluvoxamine is genuinely the right tool: OCD (especially with prior response), the sexual-side-effect-sensitive patient, and the deliberate clozapine metabolic maneuver. Where it is the wrong tool: uncomplicated depression, the polypharmacy patient, and anyone likely to stop abruptly.


Mechanism: The Short Version

Fluvoxamine is a selective serotonin reuptake inhibitor: it blocks the serotonin transporter (SERT), raising synaptic serotonin. As with all SSRIs, transporter blockade is immediate but clinical benefit takes about 4 weeks (longer in OCD); the lag is attributed to gradual desensitization of serotonin autoreceptors and downstream BDNF-mediated neuroplastic change rather than the acute rise in serotonin itself.

Two pharmacologic footnotes distinguish it. First, its potent, broad cytochrome P450 inhibition (especially CYP1A2) is not incidental; it defines the drug's clinical personality far more than any receptor selectivity does. Second, fluvoxamine is a potent agonist at the sigma-1 receptor, a chaperone protein implicated in ER stress and inflammation; this is the pharmacologic rationale behind its investigational use in inflammatory conditions, and a reminder that "SSRI" undersells what this molecule actually does.

  • Sedation: dose at night and use it

Interactions: the headline

  • Potent CYP1A2 / 2C19 / 3A4 inhibitor. Raises antipsychotics 2–10×, clozapine, theophylline, tizanidine (contraindicated), warfarin (↑INR), CYP-benzos (use LOT: lorazepam/oxazepam/temazepam), buspirone, guanfacine, caffeine
  • MAOIs contraindicated, ≥2-week washout
  • NSAIDs + SSRI → 7–15× GI-bleed risk; prefer acetaminophen, add a PPI if high-risk

Discontinuation

  • One of the worst SSRIs to stop (short half-life). Taper slowly, hyperbolically; brain zaps on a missed dose
  • Never stop abruptly; don't run out. Bridge to fluoxetine if the taper stalls

Fluvoxamine is a specialist's SSRI, not a generalist's. Choose it deliberately, for OCD, for the patient who can't tolerate sexual side effects, or for the rare case where you want its enzyme inhibition working in your favor, and it repays the choice with solid efficacy at a low price. Choose it carelessly, on a patient with a long medication list who will one day run out of pills, and it will teach you exactly why it sits where it does in the SSRI hierarchy. Respect the interactions, respect the taper, and it earns its narrow but real place in the toolkit.
