Why Nefazodone Still Matters (and Why It Almost Doesn't)
Nefazodone (Serzone) is an antidepressant most clinicians under 45 have never prescribed and many pharmacies no longer stock. Its brand was pulled from the US market by Bristol-Myers Squibb in 2004, and it was withdrawn outright in Canada, Europe, Australia, and most of the rest of the world around the same time. The reason was a single, serious problem: rare but sometimes fatal hepatotoxicity, severe enough to earn a boxed warning before the brand disappeared.
So why write a prescribing guide at all? Because generic nefazodone remains legally available in the United States, and because, hepatotoxicity aside, it does something almost nothing else in the antidepressant pharmacopeia does well. It treats depression without causing sexual dysfunction, without disrupting sleep architecture (it improves it), and without meaningful weight gain. For the specific patient who has failed or can't tolerate SSRIs and SNRIs precisely because of sexual side effects and insomnia, nefazodone is a genuinely rational, if now unusual, choice.
Nefazodone is a niche agent with a real niche. It is neither the villain its withdrawal history implies nor a first-line drug. Used in the right patient, with a frank consent conversation, baseline and interval liver monitoring, and respect for its potent CYP3A4 interactions, it is a defensible option. Used carelessly, in a patient with liver disease or co-prescribed with a drug it can push to toxic levels, it is a liability.
This is a deliberately short guide, because nefazodone is a comparatively simple drug with a small number of things you must get right. Get those things right and the rest is easy.
Nefazodone is the antidepressant you reach for when a patient needs their sleep and their sexual function preserved, has a healthy liver, isn't on a CYP3A4-sensitive drug, and understands the small-but-real hepatic risk.
Part 1: Indications: Who Is Nefazodone For?
FDA-Approved Use
- Major depressive disorder (the only approved indication)
Off-Label / Evidence-Supported Uses
- PTSD, particularly with prominent sleep disturbance and hyperarousal. Nefazodone has open-label and small-trial support and appears on second-line lists alongside mirtazapine, tricyclics, and venlafaxine. It is not FDA-approved for PTSD (only sertraline and paroxetine are), but its sedating, sleep-preserving profile fits the syndrome. SSRIs and psychotherapy often fail to fully repair PTSD-related sleep, which is where a sedating agent earns its place.
- Anxious or agitated depression: the sedating SARI profile suits patients who are keyed-up rather than fatigued.
The Patient Nefazodone Is Actually For
Efficacy across modern antidepressants is broadly similar (roughly 60% response vs ~50% placebo), so, as with the rest of the class, you choose nefazodone for its side-effect profile, not for superior efficacy. In the Cipriani network meta-analysis (522 trials, 116,000+ patients) it did not rank among the most efficacious agents (mirtazapine, escitalopram, venlafaxine, sertraline, agomelatine ranked higher). Assume roughly average efficacy and prescribe it for what it doesn't do.
The ideal candidate:
- Depression plus insomnia or fragmented sleep: nefazodone improves sleep quality and, unlike SSRIs/SNRIs, does not suppress REM or fragment sleep architecture.
- Depression in a patient who has developed, or is terrified of developing, antidepressant-induced sexual dysfunction. Nefazodone has among the lowest rates of sexual side effects of any antidepressant, consistently better than the SSRIs and dramatically better than paroxetine (AHRQ/Gartlehner meta-analysis).
- Depression where weight gain is a concern: nefazodone is essentially weight-neutral, far more favorable than mirtazapine and comparable to trazodone.
- Agitated or anxious depressed patients who need a sedating rather than an activating agent.
The cleanest way to think about nefazodone is as "trazodone strong enough to be a real antidepressant." Both are SARIs (5-HT2A antagonist plus weak reuptake inhibition); both preserve sleep and sexual function; both are weight-neutral. But trazodone is so sedating that the 300–400 mg needed for an antidepressant effect knocks most people out, which is why trazodone survives as a hypnotic at 25–150 mg and almost never as stand-alone antidepressant therapy. Nefazodone is less sedating at therapeutic doses, so you can actually push it into the antidepressant range. The price for that upgrade is the hepatotoxicity risk and the CYP3A4 interactions trazodone doesn't carry.
Who It's Not For
- Anyone with active liver disease, elevated baseline transaminases, or significant alcohol use.
- Anyone on a CYP3A4-dependent drug with a narrow therapeutic index (see Part 7).
- Fatigued, low-energy, anhedonic depression: a sedating agent is the wrong tool; reach for bupropion instead.
- A patient unwilling or unable to do baseline and periodic liver monitoring.
Part 2: Before You Start: Workup and Candidacy
Nefazodone's entire pre-start workup revolves around the liver.
Baseline Assessment

No routine ECG, renal panel, or metabolic screen is required specifically for nefazodone: it is not cardiotoxic in the tricyclic sense and carries no meaningful QT signal.
The Consent Conversation
Because of the boxed warning, document a genuine informed-consent discussion.
- Life-threatening liver failure is rare: historical estimates for liver failure resulting in death or transplant are on the order of 1 in 250,000–300,000 patient-years of treatment (roughly 3–4x the background rate of idiopathic liver failure). Milder, reversible transaminase elevations are more common.
- Instruct the patient to stop the drug and call immediately for the warning signs of hepatic injury: jaundice (yellow eyes/skin), dark urine, pale stools, right-upper-quadrant pain, unusual fatigue, anorexia, or nausea that won't quit.
- The great majority of serious injury declares itself in the first 6 months.
Symptom vigilance may matter as much as scheduled labs. Idiosyncratic hepatic injury can develop between routine blood draws, so the patient's ability to recognize jaundice and dark urine and act on it the same day is a core part of the safety plan, not an afterthought. If a patient can't be relied on to do that, reconsider the drug.
Part 3: How to Start and Dose
Formulation
Oral tablets, available as generic nefazodone in the US (common strengths 50, 100, 150, 200, 250 mg). There is no long-acting formulation. Because the parent compound is short-lived, nefazodone is dosed twice daily.
Starting Dose and Titration
- Start: 100 mg twice daily (200 mg/day total). In the elderly, start at 50 mg BID.
- Titrate up in ~100–200 mg/day increments no more often than weekly, guided by response and tolerability (chiefly sedation and dizziness).
- Effective antidepressant range: 300–600 mg/day, given BID. Most responders land at 300–500 mg/day. Many patients are under-dosed at 200 mg/day and wrongly written off as non-responders; push to a genuine therapeutic dose before abandoning the trial.
- Allow 4–6 weeks at a therapeutic dose before judging efficacy.
Why BID Dosing
Nefazodone's parent half-life is short (on the order of 2–4 hours), which is why the daily dose is split. Its active metabolites (including the serotonergic mCPP and hydroxynefazodone) persist somewhat longer. Split dosing also softens the peak-related dizziness and sedation. Some clinicians weight the larger portion toward evening to harness the sleep benefit and minimize daytime grogginess.
The two most common self-inflicted failures with nefazodone are stopping too early because of first-week dizziness and sedation that would have faded, and never reaching a therapeutic dose. Warn the patient the first week or two may feel woozy, start low, and titrate with intent toward 300–500 mg/day.
Part 4: Monitoring
Liver Function: The Whole Game
There is legitimate debate about how much scheduled LFT monitoring changes outcomes (idiosyncratic injury can outrun any practical schedule), but given the boxed warning the defensible, guideline-consistent approach is:
- Baseline LFTs before the first dose (and don't start if elevated).
- Periodic LFTs during treatment: a reasonable practical cadence is roughly every 3–6 months, weighted toward the first 6 months when most injury appears, plus any time hepatic symptoms arise.
- Stop nefazodone if AST or ALT rises to ≥3x the upper limit of normal, or at the first sign of clinical hepatic injury. Do not rechallenge a patient who has had a nefazodone hepatic reaction.
Pair the labs with the standing instruction to report jaundice, dark urine, pale stools, RUQ pain, anorexia, or unexplained malaise immediately: symptom-driven detection is your real-time safety net between draws.
Clinical Monitoring
- Sedation and orthostatic dizziness, especially early and in the elderly (alpha-1 blockade leading to orthostatic hypotension leading to falls).
- Sleep quality: the intended benefit; confirm it's helping.
- Sexual function: ask; it's a selling point of this drug and a reason patients stay on it.
- Standard suicidality monitoring per the class boxed warning, especially in patients under 25 and early in treatment.
Part 5: Side Effects and How to Manage Them
Nefazodone's side-effect profile is, ironically, one of the gentlest among antidepressants; the hepatotoxicity is the outlier that dominates its reputation.
The Favorable Profile (Why Anyone Still Uses It)
- Sexual dysfunction: among the lowest of any antidepressant. The 5-HT2A blockade means it doesn't produce the anorgasmia and low libido that plague SSRIs and SNRIs. Meta-analyses rank it well below paroxetine and below the SSRIs generally. This is a legitimate switch target for a patient with SSRI-induced sexual dysfunction.
- Sleep: improved, not disrupted. Unlike SSRIs (which suppress REM and can fragment sleep), nefazodone tends to improve sleep continuity and quality, a direct consequence of 5-HT2A antagonism.
- Weight: essentially neutral. Far more favorable than mirtazapine; comparable to trazodone.
Common Nuisance Effects (Dose-Related, Often Early)

The Serious One: Hepatotoxicity
Covered in Part 6. In routine visits, the side effect that changes management is any hint of liver injury: treat it as a stop-the-drug event, not a "watch and wait."
If your goal in switching a patient off an SSRI is to rescue their sexual function and sleep, several agents help, but each is partial. Bupropion protects sexual function but is activating (wrong for the insomniac). Mirtazapine protects sleep but causes weight gain. Nefazodone is the one agent that delivers preserved sexual function and sedation/sleep benefit and weight neutrality simultaneously, which is exactly, and only, why you'd accept its hepatic risk.
Part 6: Toxicity and the Boxed Warning
Hepatotoxicity: The Reason for the Boxed Warning
This is the defining safety issue and the reason nefazodone was pulled worldwide.
What it is: idiosyncratic (not dose-dependent), potentially fulminant hepatocellular injury, ranging from asymptomatic transaminase elevation to acute liver failure requiring transplant or resulting in death.
How common: liver failure leading to death or transplant is estimated at roughly 1 in 250,000–300,000 patient-years, rare, but several-fold above the background rate, and enough to drive a boxed warning and global brand withdrawal. Milder reversible LFT elevations are more frequent.
When: the majority of serious cases surface within the first 6 months of treatment.
What to do: stop immediately for transaminases ≥3x ULN or any clinical hepatic injury. Do not rechallenge. Never start in a patient with pre-existing liver disease or elevated baseline LFTs. Educate the patient to self-identify jaundice, dark urine, pale stools, or RUQ pain and stop the drug the same day.
Overdose
Nefazodone is relatively benign in overdose compared with tricyclics. Reported effects are largely extensions of its pharmacology (sedation, nausea, hypotension), and it lacks the cardiotoxic, seizure-prone lethality of TCAs. There is no specific antidote; management is supportive (airway, blood pressure, cardiac monitoring). As with any serotonergic drug, watch for serotonin syndrome if co-ingested with other serotonergic agents, though nefazodone's intrinsic serotonin-syndrome risk as monotherapy is low.
What Nefazodone Does Not Do
- No meaningful QT prolongation or tricyclic-type cardiotoxicity.
- No seizure signal comparable to bupropion.
- No significant weight or metabolic toxicity.
Part 7: Drug Interactions: The Second Thing You Must Get Right
After hepatotoxicity, the interaction profile is the reason nefazodone requires care. Nefazodone is a potent inhibitor of CYP3A4. This is not a footnote: it is a prescribing-defining property.
CYP3A4 Inhibition: Nefazodone Raises the Levels of 3A4 Substrates
By inhibiting CYP3A4, nefazodone can push co-administered 3A4 substrates to toxic concentrations. Key offenders:
- Certain statins (simvastatin and lovastatin especially): markedly elevated levels, raising rhabdomyolysis risk. Avoid the combination. If a statin is needed, use one not dependent on 3A4 (e.g., pravastatin, rosuvastatin) at conservative doses.
- Triazolobenzodiazepines (triazolam, alprazolam, midazolam): greatly increased levels and sedation. Reduce dose substantially or avoid.
- Buspirone: levels rise sharply; reduce dose.
- Carbamazepine: the interaction runs both ways: carbamazepine (a 3A4 inducer) can slash nefazodone levels, while nefazodone raises carbamazepine. A problematic pair; avoid or monitor closely.
- Cyclosporine, tacrolimus, some calcium-channel blockers and antiarrhythmics, and other narrow-index 3A4 substrates: monitor or avoid.
- Historically: terfenadine, astemizole, cisapride: combinations that could cause fatal arrhythmias (these drugs are now off-market, but the principle stands for any QT-prolonging 3A4 substrate).
Before you prescribe nefazodone, run the patient's full medication list against CYP3A4. The single most dangerous, most preventable nefazodone error is co-prescribing it with simvastatin or lovastatin (rhabdomyolysis) or a triazolobenzodiazepine (profound sedation). Reconcile before the first dose: treat it with the same discipline you'd apply to lithium and NSAIDs.
Other Interactions
- MAOIs: contraindicated; observe the standard 14-day washout in both directions (serotonin syndrome risk).
- Other serotonergic agents (SSRIs, SNRIs, triptans, tramadol, linezolid): additive serotonin-syndrome risk, low with nefazodone alone, higher in combination.
- CNS depressants (alcohol, benzodiazepines, opioids, antihistamines): additive sedation.
- Additional hepatotoxins / heavy alcohol: compound hepatic risk; avoid.
Nefazodone has minimal CYP2D6 effect, so, unlike fluoxetine and paroxetine, it doesn't meaningfully raise levels of 2D6 substrates.
Part 8: Special Populations
Pregnancy
Human data are limited. There is no strong signal of major teratogenicity, but the evidence base is thin and the hepatotoxicity risk applies in pregnancy as elsewhere. Better-characterized antidepressants (e.g., sertraline) are generally preferred; reserve nefazodone in pregnancy for cases with a clear, individualized risk/benefit rationale and specialist input.
Lactation
Data are sparse. Most authorities advise against nefazodone during breastfeeding when safer, better-studied alternatives (e.g., sertraline) exist.
Elderly
- Start low, go slow: e.g., 50 mg BID, then titrate, but still aim for a therapeutic dose; don't leave older patients subtherapeutic out of timidity.
- The dominant practical risk is orthostatic hypotension and sedation leading to falls (alpha-1 blockade). Check orthostatic vitals; counsel about rising slowly.
- Scrutinize the med list even harder for CYP3A4 substrates; polypharmacy is the rule in this group.
Renal Impairment
Nefazodone is hepatically cleared with minimal renal elimination, so mild caution in renal disease is generally sufficient; renal function alone does not dictate a dramatic dose adjustment.
Hepatic Impairment
Avoid. Pre-existing liver disease or elevated baseline transaminases is effectively a contraindication given the boxed warning.
Children and Adolescents
No FDA approval for pediatric depression (only fluoxetine and escitalopram carry that). Nefazodone carries the class boxed warning for suicidality in patients under 25. Use in youth is off-label, not first-line, and rarely justified given the hepatic concern.
Part 9: Discontinuation
Nefazodone's short half-life might suggest a rough discontinuation, but in practice its withdrawal profile is relatively mild, milder than the notorious short-half-life offenders (paroxetine, venlafaxine, duloxetine, desvenlafaxine). It does not appear among the worst discontinuation-syndrome agents.
- A taper over ~1–2 weeks is generally sufficient and comfortable; go longer if the patient has been on a high dose for a long time.
- Watch for the usual mild serotonergic discontinuation symptoms (dizziness, flu-like feelings, mood wobble) and slow the taper if they appear.
- If you are stopping for hepatotoxicity, stop immediately: the gradual-taper logic does not apply to a safety discontinuation.
Part 10: Nefazodone vs the Alternatives
The honest framing: nefazodone competes on side-effect profile, and for almost every one of its advantages there is a safer drug that delivers part of the package. Its case rests on delivering the whole package at once.
- vs Trazodone (its closest cousin): same SARI mechanism, same sexual-function and sleep advantages, same weight neutrality. Trazodone is a cheaper, more available generic with no boxed hepatotoxicity warning and no CYP3A4 problem, but it's too sedating to reach antidepressant doses in most people (hence its life as a hypnotic). Nefazodone's edge: it can actually be dosed as a real antidepressant. Trazodone's edge: safety and availability.
- vs SSRIs / SNRIs: nefazodone wins decisively on sexual dysfunction and sleep; SSRIs/SNRIs win on safety, familiarity, cost, and evidence base, and carry no hepatic boxed warning (though note duloxetine carries its own hepatotoxicity caution). Efficacy is comparable.
- vs Bupropion: bupropion also spares sexual function (and may enhance it) and causes no weight gain, with a far better safety story and no liver warning. But bupropion is activating; it's the wrong choice for the insomniac, anxious, or agitated patient. Nefazodone owns the "sexual-function-sparing sedating antidepressant" slot bupropion can't fill.
- vs Mirtazapine: both sedating, both sleep-improving, both low on sexual dysfunction. Mirtazapine's Achilles' heel is weight gain; nefazodone is weight-neutral. Mirtazapine has the cleaner safety profile. Choose nefazodone over mirtazapine chiefly when weight gain is unacceptable.
Why it's essentially abandoned: the boxed hepatotoxicity warning and worldwide withdrawal, the CYP3A4 interaction burden, the availability of trazodone (nearly the same benefits without the liver risk), and the general rule that safer wins when efficacy is a wash. None of that makes nefazodone wrong for a carefully chosen patient; it makes it a considered second- or third-line move rather than a default.
Mechanism: The Short Version
Nefazodone is a serotonin antagonist and reuptake inhibitor (SARI), the same class as trazodone. Its actions:
- Postsynaptic 5-HT2A receptor antagonism: the signature effect. Blocking 5-HT2A is thought to underlie both preserved sexual function (SSRIs stimulate 5-HT2A, driving sexual dysfunction and apathy) and improved sleep (5-HT2A antagonism reduces 5-HT2A-mediated wakefulness and normalizes sleep architecture).
- Weak serotonin (and modest norepinephrine) reuptake inhibition: the antidepressant "reuptake" component, weaker than an SSRI's.
- Alpha-1 adrenergic blockade: accounts for orthostatic hypotension, dizziness, and some sedation.
- Antihistaminic activity: modest, contributing to sedation (less than trazodone or mirtazapine at equivalent effect).
- Active metabolites, including mCPP (serotonergic) and hydroxynefazodone, extend and shape its serotonergic activity beyond the short parent half-life.
The net picture: enough serotonergic antidepressant action to work, with 5-HT2A blockade layered on top to cancel the sexual and sleep penalties that pure serotonin reuptake inhibition imposes.

- CYP3A4 inhibitor: the big preventable errors are a statin (rhabdo) or a triazolobenzo (oversedation). MAOIs: 14-day washout
- Relatively safe in overdose (sedation/hypotension; not TCA-like)
- Mild discontinuation: taper 1–2 weeks (immediate stop if hepatic)
- Avoid in liver disease and pregnancy/lactation unless the rationale is compelling
Nefazodone is a cautionary tale and a useful tool in the same package. Its worldwide withdrawal for hepatotoxicity was appropriate given the availability of safer alternatives, but it did not turn a working antidepressant into a poison. For the uncommon patient who needs sleep and sexual function protected, whose weight can't afford mirtazapine, who is too anxious for bupropion, and whose liver is healthy and medication list clean of CYP3A4 traps, generic nefazodone remains a rational, if deliberately chosen, option. Prescribe it the way it demands to be prescribed, with a baseline liver panel, a real consent conversation, a clean interaction check, and a patient who knows to call the moment their eyes turn yellow, and it does exactly what almost nothing else in psychiatry does at once.
