Why Paliperidone Matters
Paliperidone is risperidone's active metabolite: literally 9-hydroxyrisperidone, the molecule your patient's liver makes when you prescribe risperidone. Janssen isolated it, patented it, wrapped it in an osmotic-release tablet, and launched it in December 2006 as Invega just as risperidone was going generic. The psychiatric press was unimpressed. The Carlat Report's verdict in 2007 was blunt: "Invega looks like Risperdal without drug-drug interactions, but with more QT interval prolongation, more tachycardia, possibly more EPS, and the same amount of hyperprolactinemia. Not a very pretty picture."
That skepticism was earned. Janssen never ran a head-to-head trial against risperidone (the parent drug, sitting right there), and a Cochrane review had to conclude it was unable to determine if paliperidone has any advantages or disadvantages compared to its well-known parent compound. Approval rested on placebo-controlled data. The oral tablet was, and largely remains, a patent-extension exercise.
So why does paliperidone still deserve a place in your armamentarium? Three genuine, non-marketing reasons:
- It bypasses the liver. Paliperidone is largely renally cleared (about 80% recovered in urine, roughly 59% of the dose excreted unchanged), with no single CYP450 pathway dominating its metabolism. That erases risperidone's biggest headache, the CYP2D6 interactions that swing risperidone levels 2.5-fold, and makes it genuinely safe in cirrhosis and hepatitis.
- It is the only antipsychotic FDA-approved for schizoaffective disorder. Not a huge evidence base, but a real regulatory distinction when you're documenting.
- The long-acting injectable family is best-in-class for interval. Invega Sustenna (monthly) needs no oral overlap, and Invega Hafyera doses every six months: two clinic visits a year. That last formulation has quietly rehabilitated the oral generic as a rational stepping-stone.
Oral paliperidone is a competent, once-daily D2/5-HT2A antipsychotic whose real value is situational: pick it for the patient on hepatically cleared polypharmacy, the patient with liver disease, the patient with schizoaffective disorder, or the patient you may eventually move to a long-interval depot. Just don't believe the "6 mg and done" story: most patients need 9 to 12 mg, and the side effects climb with the dose.
A modern take, from a Carlat 20-year retrospective: "Now that paliperidone is available as a six-month injection (Invega Hafyera), I'm more likely to start the oral, generic paliperidone when treating schizophrenia, knowing that the six-month product is out there if we need it."
Part 1: Indications: Who Is Paliperidone For?
FDA-Approved Uses
- Schizophrenia (oral ER, LAIs; adults and adolescents 12–17)
- Schizoaffective disorder, as monotherapy or adjunct to mood stabilizers/antidepressants. Paliperidone is the only antipsychotic FDA-approved for this indication.
What It Is Not Approved For
Bipolar mania: failed FDA approval. This is important and often misremembered. In a network meta-analysis of 42 RCTs (~11,400 patients), paliperidone actually ranked 3rd for antimanic efficacy (effect size 0.71, behind risperidone 0.82 and haloperidol 0.74). But the registration trials were inconsistent, and the FDA declined approval. If you use it for mania, you are prescribing off-label, with a reasonable but non-approved evidence base, at an optimal dose around 9.1 mg/day.
Bipolar depression: off-label, thin data. If you want a renally cleared or metabolically lighter agent for the depressed pole, lurasidone is FDA-approved for bipolar depression and is the better-evidenced choice.
Where Paliperidone Genuinely Shines
- The patient with liver disease. Cirrhosis, active hepatitis, fatty liver, or on hepatotoxic co-medications: paliperidone needs no dose adjustment because it isn't hepatically metabolized.
- The patient on CYP2D6-active polypharmacy. Fluoxetine, paroxetine, duloxetine, bupropion, and quinidine all raise risperidone levels 2.5 to 2.8-fold and would force a dose cut. Paliperidone doesn't budge. If your patient needs fluoxetine for comorbid OCD or depression, paliperidone plus fluoxetine is cleaner than risperidone plus fluoxetine.
- The patient who needs adherence assurance. The LAI franchise (monthly Sustenna, 6-month Hafyera) is the strategic reason to start oral paliperidone at all; it gives you a defined escalation path.
- Schizoaffective disorder, especially the depressed subtype.
- Treatment-resistant schizophrenia after 1 to 2 failed trials (not a clozapine substitute, see Part 10). In a head-to-head RCT it matched olanzapine on symptoms at roughly half the milligram burden and with less waist-circumference gain.
Where Paliperidone Is a Poor Choice
- Acute agitation or severe anxiety. The OROS tablet releases over 24 hours with a peak at ~24 h: smooth, but too slow to quell an acute crisis. Immediate-release risperidone often calms an agitated patient the same day; oral Invega will not.
- Patients who cannot swallow an intact tablet. The OROS shell must stay whole: no crushing, no chewing, no feeding tube. Use an alternative (or an LAI) instead.
- Patients with baseline QT prolongation or high cardiac risk: paliperidone adds ~12 msec and 12 to 14% tachycardia. Prefer aripiprazole, quetiapine, or lurasidone.
- Prolactin-sensitive patients (young women concerned about menstrual/fertility effects; anyone with prior galactorrhea): this is a strong, sustained prolactin raiser.
The honest one-liner is "paliperidone is risperidone with the liver taken out of the equation." If your reason to pick it isn't CYP2D6 interaction, liver disease, schizoaffective disorder, or a planned path to a long-interval depot, you should probably just use generic risperidone: it's cheaper, crushable, comes as a liquid, and works faster in the acute setting.
Part 2: Before You Start: Workup
Standard second-generation antipsychotic baseline, with two paliperidone-specific twists (an ECG and renal function).

You do not need baseline or routine LFTs for drug-clearance purposes, a genuine advantage over risperidone. (Check them if the patient's medical status otherwise warrants.)
Candidacy Checks
- Can the patient swallow a tablet whole? If not, this drug is off the table (oral).
- Is the eGFR ≥60? If not, the dose ceiling drops sharply (Part 8).
- Any personal/family history of long QT, torsades, or sudden cardiac death? Any QT-prolonging co-meds? If yes, reconsider the agent.
Part 3: How to Start and Dose
Oral Extended-Release (Invega)
- Start: 6 mg once daily. Can be taken with or without food (bioavailability is food-independent, unlike lurasidone). No initial titration is required pharmacologically.
- Do not believe "6 mg and done." Janssen marketed "start and end with 6 mg, no dose adjustment needed." In the two pivotal trials, Invega 6 mg was less effective than olanzapine 10 mg, and researchers had to push Invega to 12 mg to match it.
- Typical effective range: 9 to 12 mg once daily. Titrate in 3 mg steps, no faster than every ~5 days (steady state is reached in ~4 to 5 days given the ~24-hour half-life). A clean schedule: 6 mg for ~5 days, then 9 mg for ~5 days, then 12 mg.
- Maximum: 12 mg/day. But mind the EPS penalty (below).
- Off-label acute mania: optimal dose ~9.1 mg/day.
- Treatment-resistant schizophrenia: effective mean dose ~10.7 mg/day in trial data.
The tablet is an OROS osmotic pump (same technology as Concerta): a rigid shell that pushes drug out through a laser-drilled hole. It must be swallowed whole: do not crush, split, or chew. Patients will sometimes see the empty "ghost" shell in the stool; reassure them the drug was absorbed.
EPS incidence: ~10% at 6 mg (about the same as placebo, 11%) to ~26% at 12 mg, a roughly 2.5-fold jump. So the doses you actually need for efficacy (9 to 12 mg) are exactly the doses where movement problems emerge. Titrate deliberately, and don't reflexively add an anticholinergic at 12 mg; first ask whether you can hold at 9 mg.
Long-Acting Injectable: Invega Sustenna (Monthly)
Paliperidone palmitate depot. Its signature advantage over Risperdal Consta: it works immediately, no 3-week oral overlap required, because the loading doses front-load exposure.
- Loading (do not skip): 234 mg IM in the deltoid on Day 1, then 156 mg IM on Day 8 (both deltoid). Skipping the load is the classic reason Sustenna "doesn't work."
- Maintenance: 117 mg IM every 4 weeks (deltoid or gluteal), roughly equivalent to ~6 mg oral. Range 39 to 234 mg monthly by response.
- Cost (order of magnitude): loading ~$3,000; maintenance ~$1,000/month. (Risperdal Consta ~$900/month; haloperidol decanoate ~$15/month.)
Long-Acting Injectable: Invega Hafyera (Every 6 Months)
- Every-6-month paliperidone palmitate. Patient must be stabilized on Sustenna (or the 3-month Trinza) first before converting.
- The strategic value: 2 injections a year. This is the formulation that makes the whole paliperidone family worth engaging with.
- Tradeoff: less frequent contact means fewer chances to catch early relapse or emerging side effects; reserve for engaged, stable patients.

The rational paliperidone pathway is a ladder: oral generic (test tolerability and response), then Sustenna monthly (adherence plus immediate onset), then Hafyera every 6 months (for the stable, engaged patient). Starting the cheap oral generic is defensible precisely because the six-month rung exists at the top.
Part 4: Monitoring
Metabolic (the Class Workhorse)

Paliperidone-Specific
- Prolactin: at baseline, then check when symptomatic: galactorrhea, amenorrhea, gynecomastia, or sexual dysfunction. Elevations are substantial and can persist even after dose reduction.
- Renal function (creatinine/eGFR): baseline and periodically (annually if stable; sooner if renal status changes), because clearance and your dose ceiling ride on it.
- ECG / QTc: baseline for everyone; repeat if dose >12 mg, cardiac or QT risk factors, QT-prolonging co-medication, or new cardiac symptoms. Routine serial ECGs are not required in low-risk patients.
- Heart rate: check at visits; tachycardia runs 12 to 14% (about double placebo) and is a paliperidone quirk not seen with risperidone.
- LFTs: not needed for the drug.
Sustenna post-injection: unlike olanzapine (Zyprexa Relprevv), paliperidone LAIs do not carry a post-injection delirium/sedation syndrome and do not require the 3-hour observation window.
Part 5: Side Effects and How to Manage Them
Governing principle: paliperidone's side effects are dose-dependent (EPS, prolactin, QT all climb with the milligrams) and mostly manageable, but the dose you need for efficacy is the dose that produces them, so manage proactively.
Extrapyramidal Symptoms (EPS)
Parkinsonism (tremor, rigidity, bradykinesia), acute dystonia, and akathisia. Incidence ~10% at 6 mg to ~26% at 12 mg. Dystonia in particular is more common with paliperidone/risperidone than with many peers.
Management, in order:
- Reduce the dose if clinically feasible: the single most effective move.
- Acute dystonia (oculogyric crisis, torticollis, laryngospasm): benztropine 1 to 2 mg IM/IV STAT, resolves in 5 to 30 minutes. This is an emergency; treat first, ask later.
- Parkinsonism: anticholinergic, benztropine 0.5 to 2 mg BID or trihexyphenidyl 2 to 5 mg BID; or amantadine 100 mg BID (bonus: amantadine also lowers prolactin).
- Akathisia: propranolol 20 to 40 mg BID (up to ~30 to 90 mg/day) is first-line and beats anticholinergics here; add a benzodiazepine (clonazepam 0.5 to 1 mg) if needed. Anticholinergics alone don't help akathisia.
Do not treat akathisia with benztropine: it's the wrong tool and won't work. Akathisia calls for a beta-blocker. Dystonia and parkinsonism call for an anticholinergic. And never use anticholinergics for tardive dyskinesia; they worsen it.
Hyperprolactinemia and Sexual Dysfunction
Paliperidone is a strong, sustained prolactin raiser (the depot may sustain it even more than oral). Sexual dysfunction affects 40 to 70% of antipsychotic patients and is worse with the prolactin-raisers: paliperidone, risperidone, haloperidol. Also seen: galactorrhea, amenorrhea, gynecomastia; long-term, hypogonadism and bone-density concerns.
Management (dose reduction often fails with this drug):
- Amantadine 100 mg BID: dopamine agonist, pulls prolactin down; may also aid weight.
- Add low-dose aripiprazole 5 to 15 mg/day: a partial D2 agonist that reliably lowers antipsychotic-induced prolactin and may help negative symptoms/cognition. This is the most effective add-on strategy.
- For the sexual symptom itself: a PDE5 inhibitor (sildenafil, tadalafil) helps both sexes.
- Switch to a prolactin-sparing agent: aripiprazole, brexpiprazole, ziprasidone, or quetiapine. If prolactin is the dealbreaker, this is often the cleanest answer.
Cardiac
- QT prolongation ~12 msec above placebo (similar to ziprasidone's ~10 msec). Clinically modest, but avoid in baseline QT prolongation, personal/family history of torsades or sudden cardiac death, or when stacking QT-prolonging drugs. Watch macrolides, azole antifungals, and some antiretrovirals.
- Tachycardia 12 to 14%: usually benign; check pulse, reassure, address contributing orthostasis.
- Orthostatic hypotension: less than some atypicals, but real. Check lying/standing BP early, counsel slow position changes.
Weight Gain and Metabolic Effects
Moderate, comparable to risperidone, less than olanzapine or clozapine. In the treatment-resistant trial, olanzapine produced significantly more waist-circumference gain (p=0.030).
Management:
- Lifestyle counseling; monitor weight, glucose, lipids on the schedule above.
- Metformin: start 500 mg BID, titrate toward 1,000 mg BID (~750 mg/day is enough for benefit in some protocols). Expect ~3 to 4 kg less weight gain when started concurrently with the antipsychotic. Well tolerated (mainly GI); check bicarbonate periodically (rare lactic acidosis). Start it early in patients with cardiometabolic risk or >3% weight gain in the first month.
Other Common Effects
- Somnolence: dose-dependent; dose at bedtime, or reduce.
- Nausea: usually early and self-limited; take with food.
Tardive Dyskinesia
Atypicals carry roughly 1%/year cumulative TD risk (vs ~5%/year with first-generation agents), but paliperidone does cause it. Risk climbs with age (up to ~10-fold higher in middle-aged/elderly after a year), duration, and cumulative dose.
Management:
- Prevent: lowest effective dose; screen at visits for lip-smacking, tongue movements, choreoathetoid finger/limb movements (AIMS exam).
- If TD develops: a VMAT2 inhibitor is first-line: valbenazine or deutetrabenazine (both FDA-approved for TD). (Older dopamine-depleters like tetrabenazine and reserpine are off-label and less tolerable: depression, orthostasis, akathisia.) Reduce/switch the offending antipsychotic slowly; abrupt withdrawal can transiently worsen TD. Do not add anticholinergics.
Part 6: Toxicity and Boxed Warnings
Increased mortality in elderly patients with dementia-related psychosis: largely cardiovascular and infectious deaths, plus increased cerebrovascular events (stroke, TIA). Paliperidone is not approved for dementia-related psychosis. Use in that population only after an explicit risk/benefit discussion.
Neuroleptic Malignant Syndrome (NMS)
Rare, life-threatening: fever, lead-pipe rigidity, altered mental status, autonomic instability (labile BP, tachycardia, diaphoresis), with elevated CK. Management: stop the antipsychotic immediately; ED-level supportive care (aggressive cooling, IV fluids, monitoring); dantrolene and/or bromocriptine for severe cases. Restart any antipsychotic only cautiously, later, with a different agent.
Overdose
No paliperidone-specific antidote; expect sedation, EPS, QT prolongation, hypotension. Supportive care, continuous ECG monitoring, electrolyte correction, activated charcoal only if very early. Note: because paliperidone is water-soluble and renally cleared, it can be removed by dialysis in severe renal-failure scenarios, a difference from lipophilic antipsychotics.
Other Safety Notes
- Seizure threshold: lowered modestly (as with all antipsychotics); caution in seizure history.
- The "cheeking" quirk: because the OROS tablet stays intact, a non-adherent patient can hide it in the cheek and spit it out more easily than crushed risperidone mixed in food. Helpful for detecting non-adherence under supervision; a liability for covert dosing.
Part 7: Drug Interactions, Paliperidone's Best Feature
This is the whole point of the molecule. Paliperidone undergoes minimal hepatic metabolism and is largely renally excreted (about 80% recovered in urine, roughly 59% of the dose unchanged) with no single dominant CYP450 pathway, so it has essentially no clinically significant CYP450 drug interactions.

The interactions that do matter for paliperidone are pharmacodynamic, not pharmacokinetic:
- QT-prolonging drugs (macrolides, azole antifungals, class IA/III antiarrhythmics, other QT-prolonging antipsychotics): additive QT risk; avoid stacking.
- CNS depressants / alcohol: additive sedation.
- Antihypertensives: additive orthostasis.
- Strong inducers at the extreme: carbamazepine/rifampin can still modestly lower paliperidone via P-glycoprotein and minor pathways, but the effect is far smaller than on risperidone and rarely requires a dose change.
When a patient on risperidone needs fluoxetine, paroxetine, or bupropion, or has developed liver disease, you have two clean options: cut the risperidone dose and monitor, or switch to paliperidone and stop thinking about it. That substitution is the clinical niche.
Part 8: Special Populations
Renal Impairment: the Dose-Limiting Population
Because paliperidone is renally cleared, kidney function sets the ceiling. Approximate framework:

Monitor creatinine/eGFR at baseline and periodically; re-evaluate the dose whenever renal function shifts. LAIs are generally not recommended below CrCl 50 because you cannot retrieve a depot if function declines.
Hepatic Impairment: the Easy Population
No dose adjustment needed in mild-to-moderate hepatic impairment; safe in cirrhosis and active hepatitis. This is paliperidone's signature advantage, the reason to reach for it over risperidone in liver disease.
Elderly
- Water-soluble drug, small volume of distribution, relatively higher brain concentrations, compounded by age-related renal decline (CrCl can fall ~50% by 80). Result: accumulation risk. Effective drug exposure often runs 20 to 30% higher at the same milligram dose.
- Start low (3 mg), titrate slowly (every 2 to 4 weeks, not weekly). Track renal function closely; adjust for CrCl <60.
- Heightened vulnerability to falls (orthostasis, sedation, EPS), TD (up to ~10-fold), stroke/mortality in dementia (boxed warning), and hyponatremia/SIADH (check sodium if new confusion). Consider fall precautions and PT/OT.
Pregnancy
- Untreated psychosis carries substantial risk to mother and fetus; for many patients continuing an antipsychotic is the safer path. Decide collaboratively with perinatal psychiatry and OB.
- Paliperidone crosses the placenta. Third-trimester exposure risks neonatal EPS and withdrawal (abnormal tone, feeding difficulty, tremor, agitation), a class effect flagged by the FDA; arrange neonatal monitoring.
- Associated signals: possible gestational diabetes, weight gain, and limited/mixed preterm-birth data.
- LAIs complicate pregnancy management (you cannot dose-adjust a depot quickly); oral is more flexible if changes are anticipated.
Lactation
Excreted into breast milk (roughly ~1 to 10% of maternal plasma levels). Infant sedation/EPS risk is low but possible. If the mother chooses to breastfeed, monitor the infant for excess sedation, poor feeding, and tremor, in coordination with pediatrics.
Pediatrics / Adolescents
FDA-approved for schizophrenia in adolescents 12 to 17 (and schizoaffective disorder per label). Dose lower than adults, often weight-informed. EPS, metabolic effects, and prolactin elevation all apply: weight gain and prolactin are especially concerning in teens. Wrap treatment in psychosocial support, family involvement, and school coordination.
Part 9: Discontinuation
Antipsychotics should be tapered, not stopped abruptly.
Abrupt cessation can trigger involuntary movements (chorea, stereotypies) 4 to 8 weeks later. Unlike tardive dyskinesia, withdrawal-emergent dyskinesia is usually self-limited, resolving over weeks to months. A slow taper prevents it.
How to Taper
- Reduce by ~10 to 25% every 1 to 2 weeks (over ~4 to 8 weeks for oral), monitoring for rebound psychosis, akathisia, and emergent dyskinesia.
- Schizophrenia: maintenance is generally long-term/lifelong; relapse risk on discontinuation is high. Don't taper without a compelling reason and a follow-up plan.
- Schizoaffective disorder: long-term, guided by mood stability.
- Bipolar (off-label): continue ~6 to 12 months past mood stabilization, then a slow taper (1 to 2 months) is optional if indicated.
LAI note: with Sustenna and especially Hafyera, the depot self-tapers over weeks to months; relapse, if it comes, is delayed. That's protective if a patient misses an appointment, but it also means you can't quickly reverse course; plan discontinuations accordingly.
Part 10: Paliperidone vs the Alternatives

No head-to-head trial exists between paliperidone and risperidone. Bottom line: default to generic risperidone unless a paliperidone-specific reason applies.
For the depressed pole or metabolic concerns, choose lurasidone; for schizoaffective disorder, liver disease, or CYP2D6 interactions, choose paliperidone.
The belief that depots reliably beat oral for relapse is oversold: a Cochrane review of injectable fluphenazine found no relapse reduction vs oral, and real-world LAI continuation is poor (Risperdal Consta: ~44.6% at 18 months; less than 10% still on any LAI 6 months post-discharge). LAIs help selected non-adherent, engaged patients; psychosocial support matters more than the needle. Paliperidone's edge here is purely the interval: monthly Sustenna without oral overlap, and the every-6-month Hafyera.
Mechanism: The Short Version
Paliperidone is 9-hydroxyrisperidone, risperidone's active metabolite. Like other second-generation agents it is a D2 dopamine antagonist (antipsychotic effect) and 5-HT2A serotonin antagonist (the serotonin blockade loosens the dopamine blockade in nigrostriatal pathways, buying lower EPS than first-generation drugs, though paliperidone's EPS still climbs with dose). It has relatively low affinity for histamine H1, muscarinic M1, and alpha-1 receptors versus olanzapine/clozapine, hence less sedation, fewer anticholinergic effects, and less weight gain than those two, but also little sedative cover. Its high D2 potency in the tuberoinfundibular pathway explains the robust prolactin elevation.
Pharmacokinetics that define its clinical behavior:
- Half-life ~23 to 24 hours, allowing once-daily oral dosing; steady state in ~4 to 5 days.
- OROS extended release, Tmax ~24 hours: smooth levels, but too slow for acute agitation.
- Largely renal excretion (~80% in urine, ~59% unchanged), minimal hepatic metabolism: no CYP interactions, liver-safe, renally dose-limited.
- Palmitate ester depot (Sustenna/Hafyera) dissolves slowly for 4-week or 6-month release.

Who
- Pick it for: liver disease, CYP2D6-interacting polypharmacy (fluoxetine/paroxetine/bupropion), schizoaffective disorder, or a planned path to a long-interval depot
- Otherwise consider generic risperidone: cheaper, crushable, faster acute onset
- Not approved for bipolar mania (failed FDA trials) or bipolar depression
Starting (oral)
- 6 mg QD, food-independent, swallow whole (OROS, never crush)
- Don't stop at 6 mg; most need 9–12 mg; titrate 3 mg every ~5 days
- EPS: ~10% at 6 mg to ~26% at 12 mg
LAI ladder
- Sustenna: load 234 mg Day 1 + 156 mg Day 8 (deltoid), then 117 mg every 4 weeks, no oral overlap needed
- Hafyera: every 6 months, after stabilizing on Sustenna (or Trinza)
Monitoring
- Baseline: weight/BMI/waist, glucose/HbA1c, lipids, Cr/eGFR, prolactin, ECG, BP/HR (no routine LFTs needed)
- Weight monthly ×3 then quarterly; glucose ~8–12 wk then annual; lipids 3 mo then annual
- Check pulse: tachycardia 12–14%
Side effects
- Akathisia → propranolol (not benztropine). Dystonia/parkinsonism → benztropine/amantadine
- Prolactin → amantadine 100 mg BID or add aripiprazole 5–15 mg; switch to a sparing agent if it's the dealbreaker
- Weight → metformin 500 mg BID → 1 g BID, started early
- TD → VMAT2 inhibitor (valbenazine/deutetrabenazine); never anticholinergics
Don't forget
- Renally cleared: dose-limited by eGFR; liver-safe, no CYP interactions
- QT ~12 msec: avoid in long-QT/torsades history and QT-stacking drugs
- Taper, don't stop: withdrawal-emergent dyskinesia can appear 4–8 weeks after abrupt cessation
- Boxed warning: increased mortality in elderly dementia psychosis; watch for NMS
Paliperidone entered psychiatry as a patent-extension "me-too," risperidone's metabolite in a fancy bottle, launched without the one trial (head-to-head vs risperidone) that would have settled the question. Two decades on, that skeptical read still holds for the oral tablet. But the molecule earned a durable niche by subtraction: take the liver out of risperidone and you get an antipsychotic with no CYP450 interactions, safety in cirrhosis, and a clean pairing with fluoxetine and its cousins, plus the only schizoaffective approval and, at the top of the ladder, an injection you give twice a year. Prescribe it for those specific reasons, respect the renal ceiling and the dose-EPS curve, don't fall for "6 mg and done," and paliperidone does exactly what it's good for. For everyone else, generic risperidone is sitting right next to it: cheaper, faster, and easier to swallow.
