The definitive practical guide: indications, dosing, monitoring, prolactin elevation, EPS and akathisia management, drug interactions, and why risperidone remains the default first atypical against which the newer agents still measure themselves.

Why Risperidone Still Matters

Risperidone is, for many prescribers, the reflexive first atypical antipsychotic, and that reflex is largely defensible. It has been on the market since 1993, is available as a cheap generic in every formulation you'd want (tablet, orally disintegrating tablet, oral solution, and a long-acting injectable), and carries one of the longest and best-characterized track records of any second-generation agent. In the head-to-head efficacy literature it consistently sits at the top of the non-clozapine pack: in a 2023 network meta-analysis of acute mania it ranked first among atypicals (SMD ~0.82), and in schizophrenia its effect size (~0.55) matches olanzapine and beats quetiapine and aripiprazole.

So why isn't risperidone simply the answer? Because it buys that efficacy with two liabilities that define its entire clinical personality: it is the most prolactin-elevating of the commonly used atypicals, and it is among the most likely to cause extrapyramidal symptoms (EPS) and akathisia, particularly as the dose climbs above 4 to 6 mg/day. Everything practical about prescribing risperidone well flows from those two facts. Keep the dose low, respect the prolactin, watch for restlessness in the first weeks, and you have a potent, cheap, well-understood drug. Push it high, ignore the akathisia, or dismiss the sexual side effects, and you lose the patient.

The thesis of this guide

Risperidone is a first-line workhorse whose efficacy is real and whose two signature liabilities, prolactin elevation and dose-dependent EPS/akathisia, are entirely predictable and largely dose-driven. Dose it as low as the illness allows, and it earns its place as the default atypical.


Part 1: Indications: Who Is Risperidone For?

FDA-Approved Uses

  • Schizophrenia (adults and adolescents 13 to 17)
  • Bipolar I mania/mixed episodes: acute treatment as monotherapy (adults and ages 10 to 17), or as adjunct to lithium/valproate (adults only)
  • Irritability associated with autistic disorder (ages 5 to 16): aggression, self-injury, tantrums, rapidly changing moods

Evidence-Based Off-Label Uses

Antidepressant augmentation in treatment-resistant unipolar depression. In a 4-week RCT, risperidone 0.5 to 3 mg added to a failed antidepressant produced MADRS remission of 52% vs 24% placebo and response of 55% vs 33%. The signal is real but soft: it did not separate on HAM-D or CGI, the benefit tends to fade, and it comes with weight gain (~4.3 lbs vs 0.3 lbs over 4 weeks). Think of risperidone here as a short-term kick-start, not a long-term augmenter, and note that in the naturalistic STEP-BD data risperidone augmentation performed poorly. Because antipsychotic augmentation of depression carries higher mortality than antidepressant-only strategies (~0.4%/yr vs ~0.08%/yr), and risperidone sits at the higher-mortality end, consider lithium, T3, or lamotrigine augmentation first.

OCD augmentation. Low-dose D2 antagonism is one of the better-supported antipsychotic augmentation strategies in severe, classic, SSRI-refractory OCD. Effective range is 2 to 3 mg/day, lower than schizophrenia dosing. Note the paradox: antipsychotics can also worsen OCD, most notably clozapine.

PTSD with psychotic features. Among antipsychotics, risperidone has the best RCT evidence specifically when psychotic features accompany PTSD (0.5 to 3 mg). It does not help isolated PTSD sleep disturbance or nightmares: a large meta-analysis found antipsychotics cause somnolence without improving total sleep time, nightmares, or sleep quality. If sleep/nightmares are the target, prazosin has far stronger data. Aripiprazole is often better tolerated for PTSD despite weaker evidence.

Other: ADHD with prominent aggression (0.5 to 3.5 mg reduces aggression in RCTs, but it blocks the cognitive benefit of stimulants, and valproate does similar work with fewer problems); delusional disorder/delusional infestation (often first-line); steroid-induced mania when the steroid can't be tapered; short-term behavioral/agitation management.

Where Risperidone Is a Strong Choice vs. a Poor One

Strong: acute mania (top-tier efficacy, less sedating than quetiapine/olanzapine), acute schizophrenia and first-episode psychosis, a cost-conscious first atypical trial, low-dose OCD augmentation.

Poor / think twice: young men at high EPS/akathisia risk (consider quetiapine or aripiprazole); patients where prolactin matters most (adolescents with growth concerns, sexually active adults, anyone with functional decline from sexual dysfunction); elderly patients with dementia-related psychosis (boxed warning, see Part 6); patients who need stimulants for core ADHD (risperidone antagonizes the benefit); prodromal/at-risk psychosis (three trials failed to prevent conversion; metabolic risk isn't justified).

Pearl on mixed mania

In euphoric mania, risperidone is excellent. In mixed or dysphoric mania, several algorithms nudge toward quetiapine instead, because risperidone and olanzapine can precipitate a post-manic depressive switch, whereas quetiapine carries depression-prevention data. Match the drug to the pole you're worried about next.


Part 2: Before You Start: Workup and Candidacy

Risperidone's baseline workup is the standard metabolic-plus-cardiac panel that every atypical warrants, weighted toward the things risperidone specifically does.

Who Is a Poor Candidate?

Patients with Parkinson's disease or high EPS susceptibility; those with baseline QTc prolongation or uncorrected electrolyte abnormalities (prefer aripiprazole/quetiapine/lurasidone); the elderly demented patient (boxed warning); anyone who needs stimulant efficacy preserved. None of these are absolute except the boxed-warning population, but each should push you to reconsider the agent.


Part 3: How to Start and Dose

The Master Principle: Less Is More

Risperidone's efficacy plateaus early and its side effects climb steeply with dose. Above roughly 6 mg/day you rarely gain efficacy and reliably gain EPS, prolactin, and metabolic burden. Most patients do their best work between 2 and 6 mg/day.

Formulations

  • Oral tablet / ODT (M-Tab) / oral solution: the ODT and solution are useful for adherence-uncertain patients and for fine titration.
  • Risperdal Consta: the long-acting IM injectable, 25 mg IM every 2 weeks (range 25–50 mg), roughly equivalent to 2 to 4 mg oral daily. Its microsphere kinetics release almost nothing for the first ~3 weeks, so it requires a 3-week oral overlap after the first injection. Refrigerate; reconstitute with the supplied diluent immediately before injection.
  • Paliperidone (Invega) is risperidone's active metabolite in extended-release form; covered under alternatives in Part 10.

Dosing Notes

  • The 24-hour effective half-life (parent drug ~3 h, active metabolite paliperidone ~24 h) permits once-daily dosing, usually QHS to blunt sedation and orthostasis. Some patients split BID early on to reduce peak-related side effects, then consolidate.
  • Mania needs less than schizophrenia: optimal mean ~4.8 vs ~5.7 mg (a ~16% lower dose, p<0.001). Don't over-dose the manic patient.
  • Titrate no faster than clinically needed; a slow outpatient ramp (every few days) mitigates orthostasis and akathisia.
The single most useful dosing pearl

After acute stabilization in schizophrenia, actively try to come down. A 52-week RCT reduced risperidone by 25% over 4 weeks and then to ~50% of the original dose (minimum 2 mg/day) with no increase in relapse, and with significant gains in EPS (p=0.012), BMI (p=0.005), cognition (p=0.001), and negative symptoms (p<0.001). The lowest effective dose isn't a compromise; it's often the better dose.


Part 4: Monitoring: The Schedule

Risperidone monitoring is metabolic monitoring plus targeted attention to prolactin, movement, and, in the vulnerable, cardiac and orthostatic parameters.

Prolactin: Screen Clinically, Don't Reflex-Test

Risperidone elevates prolactin more than any other first-line atypical, and elevations are common and often substantial. But elevated prolactin does not equal a problem: many patients have high numbers without symptoms. So screen with questions, not blood draws.

  • Women: menstrual changes, galactorrhea, decreased libido
  • Men: decreased libido, erectile/sexual dysfunction, gynecomastia

Order a prolactin level only when screening is positive. Sexual dysfunction from prolactin-raising agents runs 40 to 70% and is a leading, under-reported cause of nonadherence: ask directly, because patients rarely volunteer it.

Movement: Ask, Because They Won't Tell You

Akathisia and early EPS are the other silent adherence-killers. Systematic questioning (or a scale like the Glasgow Antipsychotic Side-effect Scale) surfaces problems patients don't spontaneously report. Watch especially in the first days to weeks and after any dose increase.


Part 5: Side Effects and How to Manage Them

The organizing principle: risperidone's side effects are overwhelmingly dose-dependent. Your first move for almost any of them is to lower the dose. Two exceptions to lead with, prolactin and tardive dyskinesia, where a switch is often the better lever.

Extrapyramidal Symptoms (EPS) & Akathisia: Risperidone's Signature Liability

At standard doses (6 to 8 mg/day) EPS runs ~10 to 11%, comparable to older agents; at higher doses it climbs sharply. Young males are at highest risk. The forms:

  • Acute dystonia (hours to days): spasm, torticollis, oculogyric crisis. Treat with benztropine 1 to 2 mg IM/PO or diphenhydramine; resolves in minutes to an hour.
  • Drug-induced parkinsonism (days to weeks): bradykinesia, rigidity, tremor, masked facies. Lower the dose; benztropine 0.5 to 2 mg BID as a bridge.
  • Akathisia: the most important and most missed. An intense inner restlessness that drives nonadherence and raises suicide risk. This is the one you must actively screen for.

Akathisia management, stepped

  1. Lower the dose (first-line and often sufficient).
  2. Propranolol 20 mg IR 2–3×/day, titrate toward 60–120 mg/day (up to ~240), holding for pulse <60.
  3. Benzodiazepine for rapid bridge relief: clonazepam 0.5 mg BID standing + 0.5 mg q6h PRN.
  4. Switch to a lower-EPS agent (quetiapine, aripiprazole, clozapine) if refractory.
Do not treat akathisia with benztropine

Anticholinergics help parkinsonism, not akathisia or tardive dyskinesia, a common and consequential error. And never raise the antipsychotic dose to cover EPS or emerging TD: it transiently masks by saturating receptors while worsening the long-term picture.

Tardive Dyskinesia (TD)

Risperidone, like the class, carries roughly one-tenth the TD risk of typical antipsychotics, on the order of 0.5 to 0.8%/year in non-elderly patients (vs ~5% with haloperidol). Two critical caveats:

  • In the elderly, that advantage largely evaporates: TD incidence approaches haloperidol's (~5%/year). This is a major reason to avoid risperidone as a casual choice in older patients.
  • Withdrawal-emergent dyskinesia can appear 4 to 8 weeks after abrupt discontinuation (dopamine supersensitivity). Taper slowly.

Management: dose reduction; switch toward a lower-TD agent (clozapine or quetiapine are lowest); consider a VMAT2 inhibitor for established TD. Do not up-titrate.

Hyperprolactinemia

Frequent and substantial (comparable to paliperidone). Consequences when symptomatic: amenorrhea, galactorrhea, gynecomastia, sexual dysfunction, and, with chronicity, bone-density concerns.

Management: if asymptomatic, monitor, don't chase. If symptomatic, lower the dose, or add low-dose aripiprazole (a partial dopamine agonist that reliably lowers prolactin), or switch to a prolactin-sparing agent (aripiprazole, quetiapine, lurasidone).

Metabolic: Weight, Glucose, Lipids

Risperidone is a moderate metabolic offender, meaningfully worse than aripiprazole/ziprasidone/lurasidone, meaningfully better than olanzapine/clozapine. Expect roughly 1.5 to 3 kg of weight gain in trials (more in children; one autism cohort rose from the 67th to 84th BMI percentile over 24 weeks). Diabetes signal is a non-significant ~1.6-fold (vs olanzapine's 4.2-fold); lipids rise far less than with olanzapine (triglycerides ~+32 vs +105 mg/dL).

Management:

  • Diet and exercise counseling from the start.
  • Metformin is guideline-endorsed as a co-prescription for medium-risk agents (risperidone, quetiapine, paliperidone), especially in youth or with cardiometabolic risk. Started concurrently it trims weight gain by ~3 to 4 kg (dose ~750–1,000 mg/day, up to 2,000). Metformin also improves A1c, insulin sensitivity, lipids, and prolactin.
  • Alternatives: amantadine 100 mg BID, an H2 blocker.
  • Switch to a metabolically cleaner agent if gain is early and significant (≥5–7% of body weight).

Sedation

Mild-to-moderate, mostly early, usually settling over 1 to 2 weeks. Risperidone is less sedating than quetiapine, olanzapine, iloperidone, or asenapine, an advantage when daytime alertness matters. Dose QHS; lower if persistent.

Orthostatic Hypotension

From alpha-1 blockade, most relevant in the elderly and in anyone on antihypertensives, diuretics, or other alpha-blockers. Measure sitting (≥5 min) then standing (2 min); a ≥20 mmHg systolic drop (or symptoms at 10 to 20 mmHg) is significant.

Management: slow titration, split/night dosing, hydration, rise-slowly counseling, compression stockings or an abdominal binder for the fall-prone.

Cardiac / QTc

Risperidone's direct QT effect is small (~0.6 msec at 4 to 8 mg), cleaner than ziprasidone (~10 msec) and its own metabolite paliperidone (~12 msec). Routine ECG isn't required in healthy young patients. That said, all antipsychotics carry a dose-dependent sudden-cardiac-death signal, which weighs most in the elderly and frail: obtain a baseline ECG and use caution in those with cardiac disease, QT-prolonging comedications, or electrolyte abnormalities.

Neuroleptic Malignant Syndrome (NMS): Rare, Life-Threatening

Fever, rigidity, altered mental status, autonomic instability, elevated CK. Stop the antipsychotic; supportive care, cooling, hydration; dantrolene or bromocriptine if severe. Rare but must be recognized.


Part 6: Overdose, Toxicity & the Boxed Warning

The Boxed Warning: Elderly with Dementia-Related Psychosis

Increased mortality in elderly patients with dementia-related psychosis

Pooled short-term trials showed roughly a 1.6 to 1.7-fold increase in death (largely cardiovascular events and pneumonia). Risperidone, like all antipsychotics, is not approved for dementia-related psychosis.

If, after exhausting non-pharmacologic and environmental measures, an antipsychotic is genuinely necessary for dangerous agitation, use the lowest dose for the shortest time (risperidone 0.25 to 2 mg QHS), document informed consent, and monitor closely. Remember the compounding problems in this population: increased mortality, TD risk equal to haloperidol, and falls from orthostasis.

Acute Overdose

Risperidone is relatively non-lethal in isolated overdose. Expect exaggerated pharmacology: sedation, tachycardia, hypotension, EPS, and QT prolongation. Management is supportive: airway, IV fluids/pressors for hypotension, cardiac monitoring; there is no specific antidote. Activated charcoal if early and airway-protected. Watch for arrhythmia and prolonged sedation.

Sudden Cardiac Death

The class-wide, dose-dependent sudden-death signal (roughly on the order of a few excess deaths per 1,000 patient-years in vulnerable groups) is greatest in older, frailer, higher-dose patients, another argument for the lowest effective dose and a baseline ECG when risk factors are present.


Part 7: Drug Interactions

Risperidone is metabolized primarily by CYP2D6 (to the active metabolite paliperidone/9-hydroxyrisperidone), with minor 3A4 involvement. The clinically important interactions are therefore 2D6-driven.

Levels Raised (↑ side effects, ↑ EPS/akathisia/prolactin)

  • Fluoxetine, a potent 2D6 inhibitor, raises risperidone levels ~2.5 to 2.8-fold. Its inhibition lingers up to ~6 weeks after stopping (long half-life). Anticipate more akathisia; reduce the risperidone dose.
  • Paroxetine: similarly a strong 2D6 inhibitor; same caution.
  • Bupropion: also a strong 2D6 inhibitor (via its metabolites) and can raise risperidone levels substantially, similar to fluoxetine/paroxetine; anticipate more EPS/akathisia and consider a dose reduction.
  • Duloxetine and other moderate 2D6 inhibitors: smaller effects; monitor.

Levels Lowered (↓ efficacy)

Carbamazepine (a strong inducer) drops risperidone levels ~50%; expect to increase the dose. Other strong inducers (phenytoin, rifampin) act similarly.

Pharmacodynamic Interactions

  • CNS depressants (alcohol, benzodiazepines, opioids): additive sedation, usually manageable, use judgment.
  • Antihypertensives / alpha-blockers / diuretics: additive orthostatic hypotension.
  • QT-prolonging drugs: additive risk (modest for risperidone itself); caution with macrolides, azole antifungals, certain antiarrhythmics.
  • Levodopa / dopamine agonists: risperidone antagonizes them (avoid in Parkinson's).
  • Stimulants: risperidone (like haloperidol) blunts the pro-cognitive/attention benefit of stimulants, a real problem if ADHD is being treated for its core symptoms.
One non-interaction worth knowing

Because risperidone is a 2D6 (not 1A2) substrate, smoking status does not change its levels, unlike clozapine, olanzapine, and haloperidol, which drop up to ~50% in smokers. No dose adjustment is needed when a risperidone patient starts or stops smoking.


Part 8: Special Populations

Pregnancy

Data are limited but broadly reassuring relative to the alternatives; risperidone is a reasonable option when the benefit of treating maternal psychosis or bipolar illness outweighs risk, and untreated serious mental illness in pregnancy carries its own substantial risks. There is no clear major-malformation signal, though third-trimester exposure to any antipsychotic risks transient neonatal EPS and withdrawal (agitation, feeding difficulty, hypertonia/hypotonia), so coordinate delivery planning. Manage as a shared decision with obstetrics; use the lowest effective dose; avoid reflexive discontinuation, which invites relapse.

Lactation

Risperidone passes into breast milk in small amounts and is generally considered compatible with breastfeeding by most authorities; monitor the infant for sedation and poor feeding. Its prolactin effect is a maternal, not infant, consideration. Coordinate with pediatrics.

Elderly

The population that most changes the risk calculus. Beyond the boxed warning (increased mortality in dementia psychosis) and the loss of the TD advantage (~5%/year, like haloperidol), the elderly are more prone to orthostatic falls and to accumulation as CYP metabolism slows. If risperidone is used: start 0.25 to 0.5 mg, titrate slowly, target low, monitor orthostatics and ECG, and prefer aripiprazole or quetiapine where the choice is open.

Renal & Hepatic Impairment

Both increase exposure. The label advises starting at 0.5 mg BID and titrating slowly (not exceeding ~1.5 mg BID before reassessing) in significant renal or hepatic impairment. In hepatic disease specifically, paliperidone (Invega) is the cleaner choice, as it is cleared renally with minimal hepatic metabolism and no CYP2D6 dependence.

Children & Adolescents

FDA-approved for autism irritability (5 to 16), schizophrenia (13 to 17), and bipolar mania (10 to 17). Dosing is low and weight-based (autism max ~3.5 mg/day). Youth are especially vulnerable to weight gain, metabolic change, and prolactin elevation; monitor aggressively and consider co-started metformin. In autism, pair medication with behavioral intervention: combined treatment outperforms monotherapy and allowed a ~14% lower risperidone dose. Aripiprazole is often better tolerated for the same indication.


Part 9: Discontinuation

Two distinct risks govern how you stop risperidone: psychotic/mood relapse and withdrawal-emergent dyskinesia (chorea/dystonia from dopamine supersensitivity, appearing 1 to 8 weeks after abrupt cessation). Both argue for a gradual taper.

General approach: taper over weeks to months, not days. A practical method is to reduce by ~25 to 50% at intervals, then halve the remainder, watching most closely at the low end where breakthrough occurs. Reserve rapid tapers for brief exposures. Counsel patients that staying on maintenance treatment roughly halves relapse risk.

Bipolar-Specific Evidence (Recovered Mania on Risperidone/Olanzapine + Mood Stabilizer)

  • Discontinuing the antipsychotic at 2 to 6 weeks: 87% relapse.
  • Discontinuing at 6 months: 65% relapse, the same as continuing it.

Implication: hold the antipsychotic for ~6 months after a manic episode, then attempt a slow taper (e.g., stepping down every 2 weeks) if mood is stable; there's little added benefit beyond 6 months. If the first taper fails, a second attempt after 1 to 2 years succeeds roughly half the time.

If a patient misses doses or stops abruptly, watch for both rebound symptoms and withdrawal dyskinesia; reinstating and tapering slowly usually resolves the latter.


Part 10: Risperidone vs. the Alternatives

The honest headline from CATIE and the meta-analytic literature: the atypicals are far more alike in efficacy than the marketing implied; choose by side-effect fit, not efficacy claims. With that framing:

  • vs. Olanzapine: roughly equal efficacy in schizophrenia and mania. Risperidone's decisive advantage is much less weight gain and metabolic risk (olanzapine runs ~1 lb/week early, 15 to 25 lb/year, 4.2× diabetes risk). Olanzapine's edge: lower prolactin and less EPS. For most patients, start with risperidone.
  • vs. Quetiapine: risperidone is more effective (0.55 vs 0.4 effect size) and far less sedating, but quetiapine has much lower EPS/akathisia and minimal prolactin, and it is the metabolically-cautious choice for bipolar depression. Pick quetiapine for the EPS-prone or Parkinson's patient; risperidone when sedation is unwanted and efficacy is paramount.
  • vs. Aripiprazole: aripiprazole wins on prolactin (it lowers it), metabolic profile, sedation, and mortality signal; risperidone edges it on raw efficacy, especially mania (0.82 vs 0.53). Aripiprazole is the better default when weight, prolactin, or a young male's EPS risk dominate.
  • vs. Ziprasidone / Lurasidone: both are more weight-neutral. Ziprasidone carries more QT concern (~10 msec) and needs food; lurasidone needs food and brings notable akathisia/sedation but excellent bipolar-depression data. Risperidone beats both on positive-symptom efficacy.
  • vs. Haloperidol: similar efficacy in mania (0.82 vs 0.74), but risperidone causes far less EPS/TD in non-elderly patients and a gentler first-drug experience (fewer treatment-souring acute dystonias). Haloperidol wins on cost and metabolic profile, and its decanoate LAI costs ~$15/month vs ~$900 for Consta.
  • vs. Clozapine: clozapine is superior for treatment-resistant schizophrenia (after ≥2 failed trials) and for aggression/suicidality, and it uniquely lowers mortality, but it demands ANC monitoring and carries a heavier metabolic/seizure/constipation burden. Risperidone is the earlier-line agent; clozapine is the destination when trials fail.
  • vs. Paliperidone (Invega): Invega is risperidone's active metabolite in an extended-release shell. Its genuine advantages are renal clearance (no CYP2D6 interactions, safer in liver disease) and a smoother onset. But it costs more, carries more QT effect (~12 msec) and tachycardia, at least as much prolactin, and needs escalation to ~12 mg to match olanzapine (with 26% EPS at that dose). The apt summary: it is the same wine in a fancier bottle. Reach for it mainly when 2D6 interactions or hepatic disease are the deciding factor.

A Word on the Long-Acting Injectables

Risperdal Consta and Invega Sustenna are useful for selected, adherence-motivated patients, but the LAI-as-adherence-guarantee is a myth: in a VA cohort of 11,821 patients, only 44.6% stayed on Consta ≥18 months (vs 57.9% on other orals, 77.1% on oral clozapine). Many patients simply come to dislike injections. The paradoxical upside of Consta's every-2-weeks schedule is that it forces clinic contact, aiding monitoring and alliance. Choose an LAI only for a patient genuinely on board with the program.


Mechanism: The Short Version

Risperidone is a potent D2 dopamine antagonist and 5-HT2A serotonin antagonist, the defining atypical combination. D2 blockade in mesolimbic pathways quells positive psychotic and manic symptoms; 5-HT2A antagonism disinhibits dopamine in nigrostriatal areas, buying the ~10-fold lower TD risk that separates atypicals from typicals. It also blocks alpha-1 (orthostatic hypotension) and H1 (mild sedation) receptors, with relatively little muscarinic activity.

Two mechanistic facts explain its clinical fingerprint. First, its comparatively tight, longer D2 binding (unlike quetiapine's and clozapine's fast on-off) accounts for its higher EPS liability. Second, its robust D2 blockade in the tuberoinfundibular pathway, where dopamine normally suppresses prolactin, makes it the most prolactin-elevating first-line atypical. Metabolically, CYP2D6 converts it to paliperidone, the active moiety that carries much of the daylong effect.

Quick Reference

Starting

  • Generic oral risperidone. Schizophrenia: 2 mg QHS → 4 → 6 mg (rarely exceed 6). Mania: ~4.8 mg. First-episode/OCD/augmentation: lower (0.5–3 mg)
  • Baseline: weight/BMI/waist, glucose, lipids, BP/orthostatics, AIMS; ECG and prolactin if indicated
  • 24-h effective half-life → once-daily QHS

Dose philosophy

  • Efficacy plateaus ~6 mg; side effects don't. Lowest effective dose
  • After stabilization, actively try to come down: it improves EPS, weight, cognition, and negative symptoms without raising relapse

Monitoring

  • Weight monthly ×3 then q3mo; glucose/lipids at ~3 mo then periodically; AIMS at 3–6 mo then yearly
  • Prolactin: screen with questions, test only if symptomatic

Side effects

  • EPS/akathisia (signature): lower dose → propranolol → benzo bridge → switch. Benztropine treats parkinsonism, not akathisia/TD
  • Prolactin: lower dose, add aripiprazole, or switch
  • Metabolic (moderate): diet/exercise, metformin (~3–4 kg benefit), switch if early ≥5–7% gain
  • Orthostasis: slow titration, night dosing, hydration; mind the elderly

Don't forget

  • Fluoxetine/paroxetine raise levels ~2.5–2.8×; carbamazepine halves them. Smoking doesn't affect risperidone
  • Boxed warning: avoid in elderly dementia psychosis (↑ mortality; TD risk = haloperidol)
  • Mixed mania: consider quetiapine instead (post-manic depression risk)

Discontinuation

  • Taper over weeks to months; ~6 months post-mania then slow taper
  • Abrupt stop risks relapse plus withdrawal dyskinesia
  • Young males: highest EPS/akathisia risk; consider aripiprazole/quetiapine

Risperidone endures as the default atypical for good reasons: top-tier efficacy for psychosis and mania, every formulation you'd want, decades of familiarity, and a generic price. It asks only that you respect its two demands: keep the dose low enough to stay out of trouble, and take its prolactin and movement effects seriously enough to ask about them at every visit. Do that, and risperidone remains what it has been for thirty years: not the flashiest antipsychotic, but the sensible, effective, well-mapped starting point against which the newer agents still measure themselves.
